2697619 Primary Facility 2018-02-23 Routine/Complete Inspection No Violations Noted Inspected site for Application for Inactive Well Status. There appear to be 3 wells on this pad covered with steel plates and surrounded by steel fencing and concrete barriers. There is nothing above ground. As per Well Record this well had drilling completed on 9/14/13. There is 30" conductor set at 118 ft., 16" surface casing set at 378 ft. and 11 3/4" coal protective casing set at 482 ft. The well was drilled to 520 ft.
2819310 Primary Facility 2018-12-26 Site Restoration No Violations Noted On 12/26/18 a restoration inspection of the Brunazzi-Sofran Site was conducted in response to an operator request for permit termination. The following was observed:

>>The site was well vegetated with no visible erosion or other violations.

>>No active unconventional wells or servicing equipment were present on site at the time of inspection, a nearby conventional well appeared to be functional and well maintained.

>>The accessory pad had been fully restored to pre construction conditions, the access road to the adjacent conventional well was intact.

>>Various agricultural equipment, materials, and a semi with trailer appeared to be in storage on the pad.
