2763671 Primary Facility 2018-08-13 Routine/Complete Inspection No Violations Noted On 8/13/2018 at approximately 1100, I conducted a routine inspection of the Campbell Well Site located in Independence Township, Washington County. I met with Angelo Albanese on site to discuss the addition of a gravel driveway by the landowner within the site's limit of disturbance. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • Gravel access road to the site was stoned and in good condition.
  • Landowner built driveway to a shed adjacent to the access road was approximately 40 feet in length and appeared to be stable.
  • GPU on site appeared to be in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • All wells on site were clearly marked and appeared to be in good condition.
  • The site appeared to be stable with and all PCSM devices were designed as planned.

The landowner built driveway on site was redlined in the field as a minor modification to the ESCGP2 Permit.
