The Modular Aboveground Storage Structure (MASS) was observed.  According to level monitoring equipment the current volume of the MASS was 7.5'  or approx 13500 Bbl.  The floating suction was observed.  Water is pulled from near the top of the MASS using floating aluminum strainers to supply the frac.  North Country Services is the water transfer contractor.  Eric Davis (North Country) provided additional pictures of the floating strainers via email.  One of the floating strainers was observed next to the side of the MASS.  I recommended that these floats somehow be secured to keep from damaging the liner against the wall of the MASS.  Mr. Holmes responded via email that a temporary solution using angled bracing has been implemented.  No viol

2642662 Primary Facility 2017-10-02 Routine/Complete Inspection No Violations Noted On October 2, 2017 I conducted a routine inspection of the WFN10 well site located in West Finley Township, Washington County. Matt McGuire and Josh Holmes were present from HG Energy. At the time of the inspection they had just finished Stage 5 on well G (125-27361)

No viol

2647612 Primary Facility 2017-10-11 Routine/Complete Inspection Violation(s) Noted On October 11, 2017, the Department performed an inspection of the Noble/HG Energy WFN10 well pad site. The site is located in West Finley Twp., Washington County. The inspection was a routine inspection during hydraulic fracturing operations related to a reported reuse water release on site by the operator.

5 wells have been drilled to TD and are being completed on site. The five wells on site remain permitted to Noble Energy and the permit transfers have not been processed by the Department. HG Energy is completing the wells on site as a third party consultant until the well permit transfers are completed. The operator is currently hydraulically fracturing the WFN10 GHS well. The operator reported a release of reuse water to the well pad of unknown amount because of possibly failing secondary containment, on 10/10/17. During an inspection of the site by the operator stained stone was noted on the southeast corner of the well pad near the secondary containment. Conductivity for the stone and soil on the

During an inspection of the site by the operator stained stone was noted on the southeast corner of the well pad near the secondary containment. Conductivity for the stone and soil on the southeast corner of the well pad was between 4-8 mS.

The operator removed rig mats in the area and repaired two small holes in the secondary containment. The
