2549498 Primary Facility 2016-12-06 Routine/Partial Inspection No Violations Noted On 12/6/16, I performed a routine partial inspection of the Harold Haywood in response to observed activity on site and made the following observations:

•A drilling rig was being constructed on site at the time of inspection. •A large panel had been set on the pad near the juncture of the pad and access road with a chemical container, multiple metal drums, and multiple pieces of oil covered equipment. A small amount of oil had reached the pad surface. The situation was pointed out to site personnel and was then promptly remediated including removal of all visibly impacted pad material. No violation. •A coil of hose was also found off containment dripping oily material to the pad surface and was promptly removed. No violation.

2552995 Primary Facility 2016-12-16 Follow-up Inspection No Violations Noted On December 16, 2016 I performed a follow up inspection at the Howard Haywood site in response to concerns noted in a previous inspection (ID: 2549486) of the site on 12/6/16 and made the following observations:

•On 12/6/16 multiple issues with containment and minor spills were noted to site personnel and immediately remediated. •Near the junction of the site access road and the pad a large metal plate had been placed so that it was crushing and compromising the containment around multiple pallets of chemicals marked “AES Drilling Fluids”. Multiple other empty chemical storage containers were also found without any containment. This is a violation of 58 Pa. C.S. § 3218.2(d) for failing to provide containment with at least 110% capacity on regulated substances. •A spill of more than ten gallons of antifreeze was found off of containment near the drill rig. The situation was immediately communicated to site personnel for remediation. This constitutes a violation of 25 Pa. Code § 78.57(a) and 25 Pa. Code

2586086 Primary Facility 2017-04-24 Routine/Partial Inspection No Violations Noted On 4/24/17 a RTPT inspection was conducted of the Harold Haywood site. The following was observed.

>Multiple areas along the access road bear sparse or patchy vegetation increasing risk of erosion. No violation, remediation recommended.

>A topsoil pile near the pad entrance is only partially stabilized with straw. No violation, remediation recommended.

> Drilling activity was concluded at the time of inspection and removal of drilling and completion equipment was underway.

> The AST remained on site at time of inspection.

2688463 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted On 1/30/18 a routine complete of the Haywood Pad was conducted, the following was observed:

>>Substantial equipment was found stored around the site including multiple lifts and vehicles as well as numerous lights, generators, and tanks. An above ground storage tank was found on site as well. Most equipment had adequate secondary containment. Some stored equipment did not, any leaked or spilled fluids would constitute a violation.

>>Wells on site appeared to be in various stages of completion with no rig on site.

2698965 Primary Facility 2017-09-05 Administrative/File Review No Violations Noted On 9/5/17 an administrative inspection related to the Harold Haywood was conducted regarding test results related to complaint ID 328174. No evidence was found in the sample results suggesting impact to the water supply by oil and gas activity.
2722553 Primary Facility 2018-03-27 Complaint Inspection No Violations Noted On 3/27/18 a complaint inspection was conducted in relation to the Harold Haywood Well Site in response to CTS ID: 332966. Sample 3401-071 was collected for chemistry and biological analysis.
2767363 Primary Facility 2018-08-15 Routine/Partial Inspection No Violations Noted On 8/15/18 a routine partial inspection of the Harold Haywood Site was conducted in response to increased activity at the pad. The following was observed:

>>The site was in drilling phase at the time of inspection. High activity was noted on the pad. A drilling rig was active on site at the time of inspection.

>>Multiple containers of contaminated equipment were found off containment resulting in minor impacts to the pad. This constitutes a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the operator failed to control regulated materials in a manner that prevents pollution to the ground and Waters of the Commonwealth. Any equipment with the potential to cause pollution including equipment contaminated with petroleum residues should be placed and stored on containment. Runoff from improperly stored equipment will result in violations.

>>Staining found on the pad near the office trailer suspected to be petroleum based (material was hydrophobic and greasy). This constitutes a viola

2790096 Primary Facility 2018-05-01 Administrative/File Review No Violations Noted On 5/1/18 an administrative inspection related to the Haywood site was conducted to analyze results from sample 3401071 related to complaint ID 332966 collected on 3/27/18. Sample analysis found that while the water did not meet state drinking water standards no clear indicators of oil and gas impact were present.
2805691 Primary Facility 2018-11-02 Incident- Response to Accident or Event No Violations Noted On 11/2/18 a complaint inspection of the Harold Haywood well site was conducted in response an operator reported spill. The following was observed:

>>On 10/31/18 an EQT representative reported that 15 gallons of "impaired water" was spilled to the pad.

>>At the time of inspection an operator representative reported that the standing water along the bottom of the AST was found to have very high conductivity during a routine check. The impacted water was pumped out. Field testing of standing water at the time of inspection found water at the site of the initial spill remained impacted with conductivity reading at roughly 1040us. Per the representative the standing water at the time of inspection was believed to be rain water from storms after the initial spill. The impacted water at the time inspection was substantially greater than the 15 gallons initially reported and was in addition to the initial spill. This constitutes a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the opera

2831198 Primary Facility 2019-01-22 Compliance Evaluation No Violations Noted On 1/22/19 a compliance evaluation inspection of the Harold Haywood Well Site was conducted in response to violations recorded on 11/2/18 and 12/16/16. The following was observed:

>>No apparent residual impact related to spill violations from either date were observed. Containment around the AST on site may conceal further impact. No violation.

>>Violations related to non-GPU containment were remediated around time of 12/16/16 inspection and addressed with the operator at the time.

>>GPUs on site continued to lack containment at the time of inspection including on units installed post 10/2016. Violations ongoing.
