As per the one on one conversation with Jeremy Matinko any bare spots should be reseeded and mulched to establish the min 70% coverage to complete restoration. The 6 month extension that was requested has been approved with a final restoration date of 1/5/2010.

2634119 Primary Facility 2017-09-08 Incident- Response to Accident or Event In Compliance with Policy Inspection in response to a spill reported by the Operator on 9/7/2017 at 8:57pm. At the time of inspection, the Operator was actively plugging the well. The E&S plan was on site. The PPC was on site. The drill rig was placed on three feet of fill material, which appeared to be built on top of the containment. The Operator indicate that drill rid needed to be elevated due to the height of the well head. Two open top tanks and one enclosed frac tank located on site. Both open top tanks appeared to have the required 2 feet of freeboard. The Department observed two seeps adjacent to the well pad. The seeps appear to be at or near the elevation of the well pad. The material above the seeps appeared to the fill material the rig is placed on. Seep #1 and Seep #2 are identified in the attached photos. The Operator indicated that the release occurred while the rig was circulating fresh water in the production casing, which over filled the cellar with water. 2640832 Primary Facility 2017-09-27 Follow-up Inspection In Compliance with Policy Follow up inspection to a release incident that occurred on 9/7/2017. The Operator reported the release of 4 barrels of elevated water. Secondary containment was installed at the time of the incident. According to the Operators initial report, a small fraction of fluid was released to the ground. On 9/7/2017, The Operator collected water and soil samples of the fluid released and fluid from two small seeps, located on the right side of the well site. According to the Operator, the sample results do not indicate a that the fluid released was production fluid or contained any major constituent of concern other than Iron. The Operator contacted the Department on 9/26/2017 to schedule an inspection on 9/27/2017 to visually inspect the pad after the secondary containment was removed. At the time of inspection, the well was plugged. The Operator was working to remove the stone and secondary containment from the well site. During the inspection, the Department observed the secondary containment berm was installed

A Landowner Consent form was also received with the restoration report submittal (photo #1). According to this form, the access road and well pad are to remain. At time of inspection well appears to be plugged. No erected monument present. According to Department records a buried plate is installed 36" below surface. Access road rocked. No E and S issues observed. Well pad rocked. No E and S issues observed. This restoration report is approved.
