Pit contained fluids and cuttings.  Pit lining had multiple tears.  One larger tear comprimised freeboar.  Fluid had appeared to be over cut in lining at one point.  These tears have resulted in the freeboard and integrity of the liner to be in violation. 
 
PA DEP water quality specialist Rich Thomas was present during inspection.
 
Randy Reimold responded that all pits that are currently installed are scheduled for removal asap (via email)
 
The Brady 1-2H 42-129-28602 has an associated NOV for this incident.  No further violations have been issued on this well.

| WPX started the process of plugging Brady 1-2H well.  Casing and tubing had 50 psi.  Pressure was bleed off.  35 bbl. of production water was pumped into tubing.  BOP was tested to 2500 psi.  Renegade wireline was set to depth of 7500 feet.  On December 3, 2015, WPX pumped cement plug form 7500 ft. to 2700 feet by setting 1000 ft. intervals.  The plug was set by using 113 bbl. with 485 sacks of 15.2 # Class A cement (1.25 yields).

| WPX completed the process of plugging Brady 1-2H well.  On December 4, 2015 WPX cleared hole to 2700 feet.  Cut 5.5 inch casing to 2690 feet.  Ran 2 3/8 inch tubing to 2700 feet.  Pumped 54 bbls comprised of 243 sacks of 15.2 # Class A cement (1.25 yields).  Plug was set at 2700 feet to 2000 feet.  On December 7, 2015, WPX set final cement plug at 1950 to surface using 700sacks (153.5 bbl.) of class A 1.25 yield. .

On December 4, 2015 WPX cleared hole to 2700 feet. Cut 5.5 inch casing to 2690 feet. Ran 2 3/8 inch tubing to 2700 feet. Pumped 54 bbls comprised of 243 sacks of 15.2 # Class A cement (1.25 yields). Plug was set at 2700 feet to 2000 feet. On December 7, 2015, WPX set final cement plug at 1950 to surface using 700sacks (153.5 bbl.) of class A 1.25 yield. .
2438774 Primary Facility 2015-12-10 Plugging(Includes Plugged/Mined Through) No Violations Noted WPX started the process of plugging Brady 1-4H well. WPX set cement tubing at 7510 feet. C&J cementers cemented 5.5” casing from 7510 feet to 2100 feet using 122 bbl. comprised of 550 sacks of 15.2 # 1.25 Class A cement. Cement was pumped in plug intervals of 1000 feet and laying down tubing between plugs. On 12/10/15, Casing was cut at 2070 feet. C&J cement set cement plug from 2070 feet to 1780 feet using 22.5 bbl.with 100 sacks of Class A. On 12/11/15, C&J cementing set a plug from 1789 feet to surface with 140 bbl. using 630 sacks of Class A.
