The Department suggests the Operator continue to inspect the site weekly and after significant precipitation events to ensure stability of the site and the agreed upon changes to the E&S plan achieve complete vegetation.

No observation of accelerated erosion or sedimentation in relation the well pad or access road. The pits located on site had the required 2' of freeboard and appeared to be in sound condition. The Department suggests the Operator continue to inspect the site weekly and after significant precipitation events to ensure stability of the site is maintained.

2216981 2013-10-29 Pending Inspection the result of a 25 gallon spill reported to the Department on 10/28/2013. The Operator reported a release of reuse water spilled onto the gravel well pad. During the onsite inspection the Department observed a stained line surrounding the well pad and drill pit located onsite. The line appeared to be approximately 3' wide at its apparent beginning and approximately 1' wide at its end stretching in a circular pattern approximately 500' in length. At the time of inspection the Operator and Department agreed that the spill appeared to be greater than the original report of 25 gallons. The Operator agreed to recalculate the amount spilled based on the affected area.
