0341783081 2015-11-05 Production Wells No Violations Inspector Moreland and I arrived on location to inspect the facility. When conducting the inspection, we found that frack valves and Christmas tress are in place on the well heads. At this time there is no other equipment on site. The containment liners are still in place and are in good enough shape for the next phase of fracking the wells.
0894393756 2018-08-06 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad containing three properly labeled producing wells. Surface annular pressure is 0 and intermediate annular pressure is 140. Production equipment includes six 500 barrel steel tanks within their own containment, three vertical seperators, three gas production units, one line heater, a vapor recovery system, a combustor, and a compressor. Pad is gated and locked. Outslopes are vegetated. Site meets requirements of OAC 1509.072A for preliminary restoration.
1178436187 2015-04-01 Drill / Deepen / Reopen No Violations Inspector Akins witnessed the cement jobs for the conductor on this pad. I am checking the fluid level and the cement level and found that the Tin Horns are sealed and ready for the next rig to begin drilling. It is hard to get in to this site because they are repairing the bridge and paving the road at the same time. Traffic is backed up for very long blocks of time.
1183789461 2015-06-18 Drill / Deepen / Reopen No Violations Upon arrival on location I found that the crew is drilling ahead at a depth of 14584 feet, with a target TD at 19200 feet. There are no issues at this time.
1433318712 2015-06-03 Casing String Witnessed No Violations Upon arrival on location I found that the crew had ran 403 feet of 18 5/8 inch casing in a 26 inch hole. O-Tex will do the cementing. Established circulation and circulated 32 bbls of good cement returns to surface.
1434212517 2015-07-13 Casing String Witnessed No Violations I arrived just as job was complete because of last minute notice. Nomac Drilling Rig # 84 drilled a 12-1/4 inch diameter hole to 7400 ft. and set 7397 ft. of 9-5/8 inch diameter casing. O-Tex cemented with 1015 sacks of lead cement at 13.5 lb. and a yield of 1.51 and a tail cement of 1120 sacks at 15.3 lb. and a yield of 1.27 and did not have cement to surface but did have circulation thru the entire job.
1458699174 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of natural reservoir pressure and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:
6- 500 barrel production tanks

1- Combustor 2- Compressors 3- Wells producing off of natural reservoir pressure 3- Gas Production units, equipped with internal heaters and sand traps. 1- Heater treater unit 1- Low pressure separator

The site passed preliminary restoration in accordance with ORC 1509.072 A.
1523926430 2017-04-12 Production Wells No Violations I accompanied Inspector Dillion to location. Please refer to Dillions report.
1853915362 2017-04-12 Production Wells No Violations On the day of my inspection, accompanied by Inspector Randy Akins, I witnessed metal containment has six 500 barrel steel tanks, four are condensate and two are produced water. Production equipment including two flares, two compressors, two heater treaters, and three seperators on site. Well is equipped with christmas tree type welhead with open valves. Gauges show no surface casing annular and 150 psi on intermediate annular.
