1217849634 2008-07-15 Drill / Deepen / Reopen Violations Noted Case# 08-003 , Jennifer Kirko . Conference with Don Kraeger on 7-14-2008 at approx. 3:30 PM , He advised me the rig was flaring oriskany gas . The flare was causing a heavy cloud of mist which was drifting east of well . I advised him to switch drilling operations to fluid . I arrived on site on 7-15-2008 at approx. 9:25 AM , the well was circulating fluid at this time . Don Kraeger advised me they just got well killed before I arrived . They had equipment failure the night before causing the well to be killed early in the morning . I then traveled to fire dept. and met with Fire Chief Brian Leighton , He told me they had several local complaints of a gas odor . There was one written complaint of white residue on a swimming pool and fence east of well . Conf. With David Hill , he advised me he would correct any damage done from gas flare .

1968137526 2018-02-27 UrbanProduction Wells No Violations I, along with Regional Supervisor Tom Hill, inspected this well location as an evaluation of well conditions and status. At the time of this inspection, we noted the following: a wellhead with plunger-lift system and associated tank battery in separate chain-link enclosures. The well was in active production with active chart record, and production record. The well should have a gauge at the 8.625-inch casing as required by a special condition of the permit. Fittings and connections at the battery were secure. The secondary containment was intact and free from signs of leakage or problems. We walked and inspected the areas surrounding the well and battery and did not observe any indications of problems. We noted the following deficiencies: the enclosure gates or load-line valves were not locked, the load-line was missing a bull-plug, the was no spark arrestor or carbon filter as required. In addition, there was residual oil from a spill or release around the base of the stock tanks. Supervisor Hill notified David R Hill about the deficiencies, and he indicated that they would be addressed (time frame).
