0032411822 2017-03-29 Production Wells No Violations Complaint# 5561, Chief Flanigan. My inspection of this well found the well was equipped with a plunger lift system. There was a active domestic hook-up off of the production casing head. I found no compliance issues with this well.
0413812412 2018-02-12 UrbanProduction Wells Violations Noted The lease road initially utilized the landowners driveway and then was graveled up to the wellhead and tank battery. The well was located in North Kingsville Village which is designated as an Urbanized Permit Condition Area. The tank battery consisted of one (1) 140 bbl. stock tank, one (1) 35 barrels poly brine tank, one vertical separator and one (1) gas measurement meter. The tank battery was surrounded with an earthen dike and an eight (8) foot high chain link fence. The gate was not locked and was left open. The load lines were plugged, but not locked. The inspection hatch on the top of the tank was not fastened down and left open. The required charcoal filter was not present. The identifying signage was posted on the fence, but was not compliant with Urbanized Permit Condition Area rules. The gas meter had a paper chart indicating current gas sales for the production period. The load lines, for the oil and brine drains, were plugged but not locked. The well was operated as a plunger lift. The tubulars, as seen at the surface, were 8.625 surface casing, 4.5 production casing and 1.5 tubing. The 8.625 X 4.5 annular space was open without any indication of flow. The casing and tubing both had approximately 550 psi on the respective gauges. A domestic regulation manifold was connected to the 4.5 casing. The well head was surrounded with a eight (8) foot chain link fence and access gate. The access gate was unlocked and open. There was non-compliant I. D. signage posted on the wellhead fencing. In order to bring the Blackson # 1 well into compliance with OAC 1501:9-9-05 ( E ) the Owner shall: 1.) place locks on the gates and load lines, 2.) post identifying signage that is compliant with the rules for Urbanized Areas, 3.) equip the tanks with a charcoal filter, lightning arrestor and low pressure relief valve as needed. The above list corrective measures shall be completed and approved by a DOGRM Inspector by 5/2/2018. If you have any questions and /or comments, pl
1512422628 2017-12-04 Production Wells No Violations I inspected this well pursuant to Complaint # 5561. The complainant has previously reported that they felt that they were not receiving the proper royalty payments for this well, located on their property. Upon inspection, I observed the following: a wellhead with plunger-lift and associated tank battery in separate chain-link enclosures, The well was in active production. Insurance and bonding is current. Production reports were current. I I contacted David Hill and he advised that they have provided the complainants with production documentation in an attempt to resolve the dispute. I met with the complainants who raised quesitons about the veracity of the production data. I advised the complainant that they would have to resolve the royalty dispute with the owner as a civil matter. There were no noted violations.

1894829428 2018-11-08 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 413812412, regarding urban requirements for identification, locks and tanks. The well was equipped with a plunger lift assembly and in production. The well was fenced but needed a lock on the man door. There was non-compliant I. D. signage posted on the well fencing. The tank battery consisted of one 140 BBL tank, one 50 BBL poly brine tank, and a vertical separator. The tank battery fence door still needed a lock installed. The load lines were plugged, but still needed locks installed. The inspection hatch on the top of the tank was closed. The required charcoal filter was not present. The identifying signage was posted on the fence, but was not compliant with Urbanized Permit Condition Area rules.

David R. Hill, Inc. still needs to: 1.) Install locks on the man door fences and load lines 2.) Post identifying signage that is compliant with the rules for Urbanized Areas 3.) Equip the tanks with a charcoal filter, lightning arrestor and low pressure relief valve as required per OAC 1501:9-9-05 ( E ).
