At the time of this inspection, the impacted soil within the tank battery has been removed and replaced with new soil and stone. 0397194930 2018-05-31 UrbanProduction Wells Violations Noted I received a call from Gary MacIntosh, contract pumper for David R. Hill, Inc. regarding a release of brine into the dike and outside of the dike at this well location. Mr. MacIntosh requested I inspect the site regarding the cleanup. Mr. MacIntosh explained that a cleanup crew had removed the free product from the dike. I arrived on the site and at the time of this inspection, Mr. John Suhar from David R. Hill Inc. was on location. The tank valve on one of the 100 BBL tanks had come off of the tank, releasing the fluid from the tank. The amount of product released was unknown. At the time of this inspection, it was observed that the free product within the tank battery had been removed. The northwest corner of the dike had allowed brine to flow through it and toward the north area of the site. The area affected outside of the dike was a grassy, mowed area. An approximate area measured 30X60 feet was burned from the brine scald, but no free product was present. The affected area did not appear to move off location. There was an existing pond approximately 420 feet north of the tank battery site and upon inspection, no impact was observed. Mr. Suhar stated that the impacted stone and soil within the dike would be removed and replaced with new material and the affected area outside the dike would also be removed and replaced with new soil and vegetated.

The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. The well was fenced with a locked door and had legible signage. The tank battery consisted of two 100 BBL tanks, one 50 BBL tank and two vertical separators. Valves were open on the separators. The 100 BBL tank that released the product had been isolated from the rest of the production equipment. The tank battery was diked and fenced with legible signage. The tank battery was shared with well 2-4484. Lease access was stable.

1285337473 2018-02-27 UrbanProduction Wells No Violations I, along with Regional Supervisor Tom Hill, inspected this well location as an evaluation of well conditions and status. At the time of this inspection, we noted the following: a wellhead with plunger-lift system and associated tank battery in separate chain-link enclosures. The well was in active production with active chart record, and production record. The well should have a gauge at the 8.625-inch casing as required by a special condition of the permit. Fittings and connections at the battery were secure. The secondary containment was intact and free from signs of leakage or problems. We walked and inspected the areas surrounding the well and battery and did not observe any indications of problems. We noted the following deficiencies: the unit signage was not complete (requires the well owner address, and the address nearest the well location), the enclosure gates or load-line valves were not locked, the load-line was missing a bull-plug, the was no spark arrestor or carbon filter as required. In addition, there was residual oil from a spill or release around the base of the stock tanks. Supervisor Hill notified David R Hill about the deficiencies, and he indicated that they would be addressed (time frame).
1528124450 2018-06-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 397194930. I was on location with John Suhar from David R. Hill, Inc, Gary MacIntosh, contract well tender for David R. Hill, Inc, and Circle M, subcontractor for site cleanup. It was observed that Circle M was conducting the cleanup on the site within the tank battery and at the adjacent northern area outside of the dike. The oil/brine material was being removed from the site and taken to a landfill for disposal. New topsoil had been delivered to the site and will be replaced in the area outside of the dike. Also, new stone and soil will be replaced within the tank battery dike. Reinspection of the site will be needed once the entire cleanup is complete and site restored.

At the time of this inspection, the area inside the dike near the tanks still needs to have material removed, due to residual oil left from the initial release. No changes have occurred since my last inspection on 6/25/2018.

At the time of this inspection, the adjacent area to the tank battery has been seeded and mulched where the brine scalding had occurred. New grass growth has begun in that location. The dike has been repaired and rebuilt on the northwest side of the tank battery. The area inside the dike near the tanks still needs to have material removed, due to residual oil left from the initial release. I contacted John Suhar from David R. Hill, regarding the removal of the rest of the oil impacted soil and stone within the tank battery. Mr. Suhar stated that within the next couple of weeks, the rest of the material would be removed and replaced with new soil and stone.
