At the time of this inspection, we noted the following: a wellhead with pump jack system and associated tank battery. The well was in active production, with production fittings at the 4.5-inch and 8.625-inch casings. The gauge at the 4.5-inch casing read 60 PSI. The well should have a gauge at the 8.625-inch casing as required by a special condition of the permit. Fittings and connections at the battery were secure. The secondary containment was intact and free from signs of leakage or problems. We walked and inspected the areas surrounding the well and battery and did not observe any indications of problems. Production reports, bonding, and insurance was current. I notified Elaine King of Equity Oil regarding the need for a gauge at the surface casing. 1334061249 | 2012-04-07 | Production Wells | No Violations | I received phone calls from supervisor Gene Chini at 3:00 PM and 3:30 PM regarding the operator preparing to flare annulus gas from the wellhead. I arrived on site at 6:45 PM and spoke to Pat Calkins from Petro Well Oilfield Rig Services. He stated that on Monday (April 2, 2012) his company was there to swab the perforations, when the annulus on the 8.625 casing kicked off. The annulus was shut in and the maximum recorded gas pressure was 265 PSI. The well was fractured on Tuesday (April 3, 2012). The 8.625 annulus was not blown down prior to fracturing but remained closed and was monitored by the company during the fracturing process. Pat was on site, servicing the well today, and noticed gas coming to surface in a sump, 100 south east of the well. He started venting the annulus and recorded the annulus pressuer at 175 PSI. At the time of my inspection the operator had started flaring the annulus and was holding the well pressure at 125 PSI because it was starting to unload salt water. The annulus was plumbed with a 100 blow line, the discharge end of this blow line was equipped with a 90 degree elbow with a T and placed into a pit, which was excavated to contain the flare. Per Pat, at approximately 6:30 PM, the tubing pressure on the well was 50 PSI and the pressure on the 4.5 casing was 500 PSI. The sump, to the south east of the well head, had multiple areas of bubbling, some vigorous. The drainage ditch that flows north out of the sump had areas of bubbling in it as well. Pat stated that 8.625 annulus on two offset wells, the Thomas (34-007-24535-00-00) and the Nelson (34-007-24519-00-00), had started to charge up and kick off. The Nelson well was equipped with a 50 pound pop-off and the pop-off had went off. Operator put a gauge on it and then decided to plumb it into the sales line to relieve the pressure.
