1015961559 2018-02-27 UrbanProduction Wells No Violations I, along with Regional Supervisor Tom Hill, inspected this well location as an evaluation of well conditions and status. At the time of this inspection, we noted the following: a wellhead with pump jack system and associated tank battery. The well was in active production, with production fittings at the 4.5-inch and 8.625-inch casings. The gauge at the 4.5-inch casing read 60 PSI. The well should have a gauge at the 8.625-inch casing as required by a special condition of the permit. Fittings and connections at the battery were secure. The secondary containment was intact and free from signs of leakage or problems. We walked and inspected the areas surrounding the well and battery and did not observe any indications of problems. Production reports, bonding, and insurance was current. I notified Elaine King of Equity Oil regarding the need for a gauge at the surface casing. She indicated they would address it as soon as possible,
1136694753 2012-04-03 Production Wells No Violations The well owner contacted me and advised me that, after perforating the well, there was pressure on the annulus of the production casing. I advised them to cease all activity until I conducted an inspection. I found 180 psi on the surface casing. There was 60 psi on the production casing. We released pressure on production casing to 0 psi and the pressure on surface casing remained at 180 psi. I advised them that they could frac well, but monitor the surface casing pressure throughout the fracing procedure.
1334061249 2012-04-07 Production Wells No Violations I received phone calls from supervisor Gene Chini at 3:00 PM and 3:30 PM regarding the operator preparing to flare annulus gas from the wellhead. I arrived on site at 6:45 PM and spoke to Pat Calkins from Petro Well Oilfield Rig Services. He stated that on Monday (April 2, 2012) his company was there to swab the perforations, when the annulus on the 8.625 casing kicked off. The annulus was shut in and the maximum recorded gas pressure was 265 PSI. The well was fractured on Tuesday (April 3, 2012). The 8.625 annulus was not blown down prior to fracturing but remained closed and was monitored by the company during the fracturing process. Pat was on site, servicing the well today, and noticed gas coming to surface in a sump, 100 south east of the well. He started venting the annulus and recorded the annulus pressuer at 175 PSI. At the time of my inspection the operator had started flaring the annulus and was holding the well pressure at 125 PSI because it was starting to unload salt water. The annulus was plumbed with a 100 blow line, the discharge end of this blow line was equipped with a 90 degree elbow with a T and placed into a pit, which was excavated to contain the flare. Per Pat, at approximately 6:30 PM, the tubing pressure on the well was 50 PSI and the pressure on the 4.5 casing was 500 PSI. The sump, to the south east of the well head, had multiple areas of bubbling, some vigorous. The drainage ditch that flows north out of the sump had areas of bubbling in it as well. Pat stated that 8.625 annulus on two offset wells, the Thomas (34-007-24535-00-00) and the Nelson (34-007-24519-00-00), had started to charge up and kick off. The Nelson well was equipped with a 50 pound pop-off and the pop-off had went off. Operator put a gauge on it and then decided to plumb it into the sales line to relieve the pressure. At this time the operator was actively working to prep the 8.625 casing on the Thomas well for flaring. (see Thomas report for this day.) Th
1334145343 2012-04-10 Production Wells No Violations I was on site with DOGRM Inspector Jerry Kohl, Supervisor Gene Chini and Deputy Chief Bob Worstall. The natural gas coming to surface in the sump to the south east of the well has decreased. There were only two areas in the areas of water empoundment where gas could be seen coming to surface and the rate of bubbling has decreased. The annulus pressure on the well was down to 90 PSI. The flare line had been staked down and the flare was still lit.

1342554005 2012-07-06 Production Wells No Violations Equity is still flaring and producing the annular gas (Oriskany) from this well, while maintaining a maximum annular pressure of 95 psi. The well is producing into the Dominion system through a 2 tap near SR 20 while simultaneously flaring excess gas south of the wellhead to control the annular pressure. Equity is arranging to install a 4 tap to replace the current 2 system. This may allow enough gas to enter the Dominion system to safely extinguish the flare. The Clinton interval has been fraced but is not in production. The compressor went down for repairs on 7/05 and should be back in operation later today. The flare continues to be monitored 24/7.
1345741115 2012-08-15 Production Wells No Violations My inspection found a compressor pumping gas from the annulus of the 4.5 production casing with 10 psi of suction and 85 psi of pressure into the sales line. The pressure of annulus was 55 psi. The pilot light on the flare line was burning. No violations.
1377533079 2013-04-29 Production Wells No Violations Inspection found the large compressor has been replaced with a much smaller compressor . The pressure on annulus was at 20 psi . The production casing pressure was 920 psi and the pressure on the tubing was 0 psi . The compressor was operating at the time of my inspection .
1466159863 2012-04-08 Production Wells No Violations I arrived on site to find company flaring gas from surface casing. The surface casing pressure was 100 psi while they were flaring. The production casing was at 900 psi. When the surface casing was shut-in the pressure went to 180 psi. I advised the company to continue to flare until the preessure was 50 psi or less.

The pressure on the surface casing annulus, while flaring, is 100 psi. I had company worker drop pressure to 90 psi. They are still producing gas through the compressor at the same time. Wade Pol told me that the 4 tap has been installed and they were waiting for the 4 meter to hook into sales line.
