0533072020 2015-08-18 Orphan No Violations On site with Orphan Well Manager, Gene Chini, to evaluate the well and site conditions for possible well plugging by the Orphan Well program. Cement is evident between the tubing and casing and gas is leaking through the cement. See earlier inspection reports and the case file. Conneaut Township Park initially contacted the Division in 1997 to request the leaking well be plugged, Case # 97-003. The well is an area of known Shale wells. The Division of Oil and Gas and the Ohio Fuel Gas maps do not show a well at this location.

1201058012 2019-02-26 Orphan No Violations At the time of my inspection I was on location with inspector Kevin Perry. He stated that the well was dry of fluid and the well was gaged at 16 PSI. Hydro carbon well service pulled 218 feet of tubing out of well. Timco Inc. ran a collar locator down well and got to the total depth of 240 feet. On site was a service rig, one steel pit open top, one 100 BBL production tank, and a mini excavator.
1326972802 2017-03-06 Orphan No Violations Todays inspection was conducted for the Technical Team to review the well site. The water level inside the well vault is now covering the top of the well and the tubing. Bubbling is evident in the SW corner of the vault.
1443539703 2015-07-31 Orphan No Violations Conneaut Township Park initially contacted the Division in 1997 regarding this leaking gas well. The well was referred into the Orphan Well Program, complaint # 97-003. The well is in the NW corner of a gravel parking lot approximately 6 from the edge of the park drive. There is a playground located approximately 30 north of the well. This follow--up on the collar it was difficult to accurately determine the true diameter. Cemented in this casing is 2.75 diameter tubing. There is a bull plug installed in the tubing collar which is equipped with a fitting for a pressure gauge. The fitting is closed and has a plug in it. The well was metered for natural gas and the Sperian meter, equipped with piping and the filter, detected 10% LEL/0 ppm H2S at the cement around the tubing.

There is a flat weight/balance regulator in the east side of the vault that is disconnected from the well. These regulators may contain mercury and should be handled appropriately. There is an old steel service line goes into the ground from the regulator. This line used to run to an old gas grill that was located NE of the well and also to the house that is located to the south of the well, at the top of the hill. U

1471971882 2016-08-01 Orphan No Violations The Conneaut Township Park #1 well has been submitted to the Orphan Well program for possible plugging. The well is located in a parking area across from a playground. The well is leaking gas and todays inspection was conducted to check the gas level at the well site. A slight gas odor is evident in the air around the well vault. The vault is covered with a steel lid and my Sperian gas meter showed a 100% LEL/0 ppm H2S reading 4 from the lip of the north side of the steel lid. Gas was not detected by my meter at farther distances. I reviewed the status of the well plugging and possible submittal into the Landowner Pass Through Program with the Park Manager, Bruce Mitchell.
