0489997811 2013-08-12 Production Wells No Violations I was onsite to check for the possibility of runoff from the drilling pad running into a local residence pond. The property and the ponds were not clearly identified in the report and I checked some in the area. The ponds appear to have no issues and the pad is currently dry and does not show any signs of substantial runoff. A clearing has been made near the pad for what looks like a pipeline right away. An inspection will occur more thoroughly when more information is received.

1156450022 2015-01-14 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

1193744062 2013-06-06 Casing String Witnessed No Violations I was onsite to witness the 13 3/8 surface casing set at 608 be cemented by Otex. The well was circulated fully before cemented. The job was pumped as planned. The cement was scaled at 15.6ppg before it was pumped down hole. There were a total of 23 barrels of cement returned to surface. There are no violations at this time.
1338165642 2017-08-02 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #8H well is in production and is equipped with a plunger lift unit. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consisted of the following: 6 producing wells

1 Low pressure vertical separator 6 sand separators 6 gas processing units 1 combustor unit The lease road was stable. No violations were noted at the time of inspection.

1362766404 2013-07-10 Drill / Deepen / Reopen No Violations I was onsite to update the GPS coordinates for this wells as well as check the annulus pressure of these 6 wells drilled by Nomac 72. It is unsure when the wells be hydraulically fractured at this time. Currently there was a vacuum truck on location sucking water off the pad. There was also a Stonebridge crew onsite removing the rig containment that was leftover. All the wells were checked and none showed any signs of pressures on the wells. The wells are currently static and there are no violations at this time.
1371237283 2013-06-12 Drill / Deepen / Reopen No Violations I was onsite to witness the drilling by Nomac 72. This well is currently being drilled in its lateral section at 10700 feet. They plan to TD the well in 24 hours. The rig was currently slide drilling in order to get back to its drilling window. The walking of this rig will be straight ahead so it will be a quicker walk then usual following the end of this well. There are no violations at this time.
1371505121 2013-06-14 Casing String Witnessed No Violations I was onsite to witness the cementing of the 5.5 production casing set to 13382 feet. The well bore was conditioned before the job commenced. The casing was rotated and reciprocated during the job until the casing torqued up and stalled out. The job was pumped as planned and in total 20 barrels of cement was estimated to be returned to surface. Nomac 72 is going to walk the rig to the Putnam 6H well next.
1588858460 2013-07-12 Drill / Deepen / Reopen No Violations I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completions crew to come and hydraulically fracture the wells. All the six wells have well caps on them and the cellars are watertight as they have not been pumped out since the rains came through. There are no violations at this time.
1780841174 2018-11-01 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 300 psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
1944949837 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
