0707176604 2018-12-18 Production Wells No Violations I was on site in response to complaint #6031. I received the complaint on 12/13/2018 from caller who wished to remain anonymous and was concerned of dozer work at the Shaw Properties freshwater impoundment causing stability issues of the pad slope. This freshwater impoundment is located west of the Shaw Properties producing pad. DOGRM Engineer Paul Logan was on site on 12/14/2018 to access the location. He spoke to a contractor who was on site doing dozer work. The contractor was working for the landowner and was removing soil to fill in a wet area located at the base of the west slope of the pad. The contractor stated that no more than two feet of soil has been removed. The DOGRM engineer observed no stability issues on 12/14/2018. I located the disturbed area on 12/18/2018 and observed no erosional and stability issues or concerns. The complaint can be closed at this time.

0849479777 2018-12-18 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed zero psi. I performed a walk-around inspection of the pad and no violations were observed at the well or production pad at the time of inspection.
1385572308 2013-11-25 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the intermediate casing. The drilling contractor, Patterson Rig # 329, drilled a 12.25 diameter hole to 1900. There were no issues while drilling the intermediate hole. 9.625 OD casing was set at 1885 KB. A total of 14 centralizers were installed on the intermediate casing string. O-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. O-Tex adjusted the cement density, and pumped 162 bbls of cement slurry at 15.6 ppg. Cement returns to the surface were observed. An estimated 28 bbls of cement returned to the surface. The plug was displaced with 136 bbls of water. The plug landed and was pressured up on. The float held when the pressure was released. 1.0 bbls of water bled back.
1603651429 2015-11-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
