0295093823 | 2014-01-17 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 73 has drilled a 17 1/2 inch to a 510 feet and ran 500 feet of 13 3/8 inch casing (54.5 pounds per foot / inside diameter of 12.615 inches). 5 bowspring centralizers were ran on the casing string on joints 1,3,6,9, and the marker joint. O-TEX cemented the 13 3/8 inch casing with 470 sacks of Class H with 2% CaCl, 0.4% C-41P, and 1/4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 101 barrels of cement slurry, 70.5 barrels of displacement, and there was 26 barrels of cement returns to surface. They bumped the plug at 10:16 pm with 270 PSI and then bumped the pressure up to 940 PSI for the casing test. The casing test was approved, floats held, and there was approximately 1/2 barrel flowback when the pressure was released. My inspection revealed no violations at this time. |
0419695214 | 2018-10-16 | Production Wells | No Violations | The well is equipped with a plunger-lift assembly. No changes have been made to the pad production equipment since my last inspection. The surface/intermediate and intermediate/production annular gauges displayed zero psi. No violations were observed at the time of inspection. |

0576549652 | 2014-01-19 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Nomac Rig # 73 has drilled a 12 1/4 inch hole to a total depth of 1916 feet and ran 1906 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter 8.835 inches). They installed 15 bowspring centralizers on the casing string on joints 1,4,7,10,13,16,19,22,25,28,31,34,37,40, and 43. O-TEX cemented with 710 sacks of Class H 3% Gypsum, 2% CaCl, 0.8% MC-500, 0.5% C-45, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, a yield of 1.23 cubic feet per sack, and they used 5.14 gallons of mix water per sack. O-TEX pumped 155 barrels of cement slurry, 138 barrels of displacement, and there was 38 barrels of cement returns to surface. They bumped the plug at 3:40 pm with 900 PSI and then bumped the pressure up to 1400 PSI for the casing test. The casing test was approved, floats held, and there was 3/4 barrel of flowback when the pressure was released. My inspection revealed no violations at this time. NOTE: Had to wait for O-TEX to bring another pump truck from their Malvern yard because the recirculating pump on the pump truck, that was on site, would function properly. |
1069533188 | 2015-09-14 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed. |
1390159629 | 2013-01-24 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express has drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pounds per foot / inside diameter 19.124 inches). Allied cemented the casing with 250 sacks of Class A with 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 52.5 barrels of cement, pumped 39 barrels of displacement, and there was 5 barrels of cement returns to surface. My inspection revealed no violations at this time. |
1905842250 | 2017-11-16 | Production Wells | No Violations | I conducted a status check of the well and pad. The pad houses six producing wells, and the well is equipped with a plunger lift assembly. The surface-
