0038956900 2018-11-06 Production Wells No Violations The well was equipped with an open-flow assembly. The surface/intermediate annular gauge displayed readings of zero psi. No violations were observed on the production pad at the time of inspection.
0805660141 2013-12-01 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #72 drilled a 17.5 hole to a total depth of 859 measured from the kelly bushing (KB +20) and ran 19 joints of 13.375 casing to a depth of 808 (KB) with a float shoe on bottom, a float collar at 763 (KB), and 7 centralizers. Schlumberger racked in, tested the lines, established circulation, and conditioned the hole with 25 barrels of gel spacer. They then mixed and pumped 152 barrels of Class A cement, dropped the plug, and displaced the plug to the collar with 118 barrels of water. The plug landed at 660psi and the pressure was increased and held at 1008psi for 5 minutes with 6psi leak off. Pressure was bled off and the floats held. 45 barrels of cement circulated to surface. 0815472337 2013-11-25 Drill / Deepen / Reopen No Violations I was on location to inspect the move and rigging up of Nomac Rig #72. The move is progressing and the location has limited space. There are 3 live wells on the pad that have been shut in prior to the move in. Rig should be ready to spud the surface hole by the end of the week. No violations.
0847653144 2013-12-03 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Nomac Rig #72 drilled a 12.25 hole to a depth of 1961 (KB) and ran 45 joints of 9.625 casing with 2 marker joints, a landing collar, and a landing joint with 16 centralizers to 1951 (KB). The casing had a float shoe on bottom and a float collar at 1862 (KB). Schlumberger racked in, tested the lines, established circulation, and conditioned the hole with 35 barrels of gel spacer. They then mixed and pumped 150 barrels of Class A cement, dropped the plug, and displaced the plug to the collar with 141.5 barrels of water. The plug landed at 911psi and the pressure was increased and held at 1369psi for 5 minutes with 9psi leak off. Pressure was bled off and the floats held. 11 barrels of cement circulated to surface. 1637356085 2015-11-17 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
