0070602008 2014-07-13 Drill / Deepen / Reopen No Violations Top Hole Drilling augured a 26 hole to a depth of 120. Allied Oil & Gas established circulation with fresh water and pumped 247 sacks of class A cement with 2% Calcium, .125 pps Floseal and displace to a depth of 110 with 39 barrels of fresh water. There was 10 barrels of cement returned to surface.
0247858212 2015-02-24 Completion Testing No Violations I was on the Legggett pad to observe the hydrofracture stimulation of the Leggett #3-H and Leggett #8-H wells. Frac Tech Services was the stimulation contractor. Bill Rubin was the consultant for Chesapeake. They were trying to frac stage 15 of the Leggett #3-H, but water supply issues prevented have a sufficent volume of fluid. Ambient temperature was 15 F and the charge pumps were freezing. The stages were being alternated between the #3-H & #8-H wells using a zipper frac. Leggett wells # 7-H & # 2-H were previously completed utilizing this method. This is an eight ( 8) well pad and is scheduled for a total of 398 stages. At the time of this inspection 122 stages had been completed. A full pad inspection was not conducted due high pressure equipment on the pad.
1357320125 2018-09-10 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed readings of zero psi. No changes have been made to the production pad equipment since my last inspection. No violations were observed.
1412341743 2014-10-03 Casing String Witnessed No Violations I was on location to observe the placement and cementing of the intermediate 9.625 casing string. Patterson Rig #329 drilled a 12.25 borehole to 2168 MD ( 2117 TVD ) and placed 9.625 casing to 2136 GL. Schlumberger mixed a cement slurry and displaced it with 151.5 bbl. of water. A total of 801 sacks of SurfaceSet cement was used. The displacement plug was landed and bumped to 1387 psi and held for 5 - minutes. I observed approximately 35 bbl of cement returned to the surface. No violations were identified during this inspection.
1412966524 2014-10-09 Drill / Deepen / Reopen No Violations I inspected this drilling location on 10/9/2014. Patterson Rig # 329 was drilling the Leggett # 5 - H lateral @ 13250 on fluid. This is the fifth well on the pad. I performed a walk around inspection on the pad and did not identify any violations during this inspection.
1860297033 2014-10-01 Casing String Witnessed No Violations I . was on location to witness the cementing operations on the Leggett # 5. Patterson Rig # 329 drilled a 17.5 hole to 616 MD and inserted a string of 13.375 casing to 607 KB. Schlumberger established circulation and mixed a 600 sack cement slurry. The cement was circulated back to surface with 42 barrels of cement returned. The plug was landed at 211 psi and subsequently increased to 710 psi. After a 5 minute shut - in period the pressure was released and .5 barrel flowed back. All floats held. No violations were observed during this inspection.
1878400393 2017-03-02 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The #3H well is in production and consists of plunger lift assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of twelve 400 bbl production tanks, four labeled as containing produced water, and eight labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of: 12- 400 bbl production tanks, four labelled as containing produced water, and eight labelled as containing condensate 8 sand separators 8 gas processing units 1 low pressure vertical separator 1 oil stabilization unit 1 vapor recovery unit 1 horizontal separator, 1 compressor station Two bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

1891000830 2017-11-01 Production Wells No Violations I conducted a status check of the well and pad. No changes have been made to the production equipment since my last inspection. The well is equipped with a plunger-lift unit and the surface-intermediate annular gauge displayed a reading of zero psi. The lease road was in good repair. No violations were observed at the time of inspection.
