0005452834 2014-08-25 Casing String Witnessed No Violations I was on location to witness the placement and cementing of the 20 conductor casing. Top Hole Drilling drilled a 120 deep bore hole which was 26 in diameter. Three joints of 20 casing ( 94 lbs / foot ) were welded together as it was lowered into the bore hole. Openings were cut into the bottom of the casing five feet from the bottom to allow for circulation. Express Energy Services connected to the 20 casing and pumped a cement slurry of 230 sacks of Class A with 3 % CaCl2 back to the surface. I observed 15 barrels of cement return which weighed 14.9 lbs. / gallon. Water temp. - 78, pH - 7.4. No violations were observed.
0084178563 2017-03-02 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #5H well is in production and consists of a plunger lift assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of twelve 400 bbl production tanks, four labeled as containing produced water, and eight labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

12- 400 bbl production tanks, four labelled as containing produced water, and eight labelled as containing condensate 8 sand separators 8 gas processing units 1 low pressure vertical separator 1 oil stabilization unit 1 vapor recovery unit 1 horizontal separator, 1 compressor station Two bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

0507298431 2018-09-05 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediateannular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 150 psi. No changes have been made to the production pad equipment since my last inspection. No violations were observed.
1605237104 2014-10-15 Casing String Witnessed No Violations While on the Leggett # 9 - H location for the cementing of the surface casing I noticed a slight bubbling in the adjacent cellar ( Leggett # 5 - H , 34-019-2-2606 ). I contacted Chesapeake personnel who had the cellar of the Leggett # 5 - H pumped dry. A leak had developed in the side gate valve of section A of the Sweco wellhead. The pressure gauge did not register any pressure. The spindle packing was allowing a leak to pass out of the stem of the valve. With Sweco personnel on location, Chesapeake directed Sweco to correct the matter. Sweco personnel squeezed into the packing gland additional packing to fill the void. Upon refilling the valve the cellar was filled with fresh water to allow my visual inspection. The remedy sealed the valve stem. I inspected the wellhead on 10/15/2014 and did not observe any leaking. No violations were identified at this time.
1763504033 2018-11-19 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate gauge displayed zero psi and the intermediate/production gauge displayed 40 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
