Price #3 (#21183), and the T. Price #4 (#21190). Identification was posted at the tank battery and wellhead. The tank battery consists of two 210 bbl production tanks, two concrete brine vaults, and four horizontal separators. The tanks valves are sealed with a plug and locked. There is a meter box with a current seven day chart showing production. A dike is present and in good repair. The wellhead is equipped with an electric pumpjack and a domestic supply line. The access road and site are stable. The wellhead is listed as Final Restoration (FR) in the state database. The wells last reported production was in 2011 and was 0 bbls oil, 765 mfc gas, and 12 bbls brine. Status needs updated to Producing (PR) in the state database. Operator will be contacted to inquire about the lack of reported production. |

1511288927 | 2017-11-21 | Production Wells | No Violations | The purpose of my inspection was to follow up on complaint # 5668. I called and spoke with M&R Investments production superintendent Rusty Morrison on 11.01.2017 concerning the status of repairs. Mr. Morrison reported that the flow line had been replaced. At the time of my inspection I found a stable earthen dike constructed around the well head. Correct identification was present. Production valves were in the open position. No leaks were observed. The areas disturbed by flow line replacement remained with mounded earth to allow for settling. No established vegetation was present. No signs of erosion were observed. The horizontal separator for well #2-1183 was operating at the time of inspection. A current production chart was in the meter house. The stem on the dump valve had caused production fluid to leak from the assembly and onto the ground. The fluid was fresh but not mobile The area of impact was less than 1 square foot. I observed gas and production fluid leaking from the 3 PT switch assembly. The assembly was covered by a steel box that prevented any fluid from collecting on the ground. I spoke with production foreman Chris Belch concerning the status of flow line restoration and the condition of the separator. Mr. Belch stated that he would need to inquire about the status of the restoration and that he would contact lease operator Joe Lombardi to investigate the leaks and make necessary repairs. I spoke with Mr. Pitts on 11/30/2017 concerning the restoration work. Mr. Pitts stated that he would prefer to have any additional work done in the Spring after allowing the disturbed area to settle. Mr. Pitts agreed to close the complaint and would contact ODNR-DOGRM if any additional issues occurred. |

A review of RBDMS indicated the owner was current with reporting production. The well was located within an agricultural field. Access was through the farm driveway. Correct and legible identification was posted at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the closed position. No audible or visual leaks were identified. The tank battery location was shared with #2-0878, #2-1188, and #2-1190. A stable earthen dike was constructed around the equipment. The interior of the dike was free of fluid. Production equipment specific to well #2-1183 consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The production tank and brine vault were common to well #2-1183, #2-1188 and #2-1190. The separator remained in good repair. No visual leaks were observed. An expired production chart was in the meter house. Correct and legible identification was on the production tank. The tank body remained in good repair with no holes or leaks observed. The load line valve was closed, locked and plugged. The drain line valve was closed, locked and plumbed to the vault. The hatch lid was closed. The brine vault contained a minimal amount of fluid. No evidence of overflow was present. No violations were observed during this inspection. |
