0049376888 2017-08-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1774. The lease road and pad surface was composed of crushed gravel and remained in good repair. Correct identification was posted at the entrance and well head. A locked chain link fence remained around the pad. The well head was protected by jersey barriers. The cellar was backfilled with gravel. The annular gauges were plumbed at grade and identified. Water in the cellar prevented determining if the valves were open. The 9.625 gauge indicated 0psi. The 5.5 annulus gauge identified 500 psi. The 5.5 production casing was equipped with tubing and was in production. The production casing gauge indicated 313 psi. The tubing pressure gauge indicated 164 psi. No leaks were observed. Production equipment on the pad consisted of one (1) sand knock out and GPU, one (1) hydrate inhibitor tank, one (1) thermal generator, one (1) meter run and two (2) 400 bbl production fluid tanks in containment. The containment was lined with impervious material and was holding water with no sheen. Valves were closed and plugged. No violations were observed during this inspection.
0401989182 2013-12-25 Drill / Deepen / Reopen No Violations I was notified that the company was going to cement the surface casing. The drilling contractor, Nomac 43, spudded the well on 12/24/2013. The 17.50 diameter surface hole was drilled to 593. There were no issues while drilling the surface hole. Circulation was maintained throughout the drilling. The 13.375 OD surface casing was set at 570 KB. A total of 6 centralizers were installed on the surface casing string. Schlumberger was the cementing contractor. I was informed of good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.6 ppg. Schlumberger pumped 114 bbls of cement slurry at 15.6 ppg. An estimated 25 bbls of cement returned to surface. The plug was displaced with 84.75 bbls of water. The plug bumped the float collar at 4:02am. Schlumberger pressured up on the casing and help pressure for 5 minutes. The plug held and pressure was released. The pressure test on the casing and shoe will be conducted before initiating drilling of the intermediate hole.

0925110367 2013-12-23 Drill / Deepen / Reopen No Violations I was contacted today by Chesapeake Exploration. The company requested a change in the surface casing program. The permit has the depth of casing to be set at 640. The company has requested it to be changed to approximately 572 to 599 of casing depth. The company is going to isolate the fresh water formation with the surface casing. The intermediated casing will cover a salt water zone and the berea sandstone formation. The idea of setting the surface casing shallower is to separate the fresh water and the salt water formation. Tom Hill and I have agreed that the casing program can be changed to the requested depth.
0931053730 2013-08-20 Surface Facility Construction No Violations I went to the site to review the construction process of the drilling pad. The entry off SR644 was started with crushed limestone, and the access road was built, but the total length was not yet covered in stone. The drilling pad was largely built. A berm has been constructed surrounding the pad. A construction crew was still finishing the drainage, erosion and sedimentation controls. Stone had not yet been spread on the pad.
1377296187 2013-02-11 Surface Facility Construction No Violations I went to the site and conducted a pre-construction site inspection with Steve Ault, Chesapeake Field Representative.
1388430302 2013-12-27 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the intermediate casing. The drilling contractor, Nomac 43, drilled a 12.25 diameter hole to 1630. There were no issues while drilling the intermediate hole. 9.625 OD casing was set at 1624 KB. A total of 13 centralizers were installed on the intermediate casing string. Schlumberger was the cementing contractor. There as good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.6 lbs per gallon (ppg). Schlumberger pumped 127 bbls of cement slurry at 15.6 ppg. Cement returns to the surface were observed. An estimated 22 bbls of cement returned to the surface. The plug was displaced with 116.5 bbls of water. The plug did not bump the float collar. The company decided not to continue displacing the cement. Pressure was held on the cement until it was stable. The company previously estimated the 9 5/8 casing was set at 1585. The bond log proved the miscount of casing and shows the casing to be set at 1624.
1459430619 2016-03-31 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #8H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 2 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 1 - Gas processing unit (GPU) 1 - Sand separator 1 - Thermal electric generator 1 - Producing well Site fenced and man doors locked

1903840739 2017-02-01 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1774-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road was stable and drivable. No signs of erosion were observed adjacent to the road traveling to the pad. An earthen dike perimeter is erected around the well pad. A chain link fence with barbed wire top is constructed around the pad to prevent unauthorized entry. An Emergency Shut Down (ESD) button was adjacent to the main entrance gate. The main entry gate and escape doors positioned around the perimeter were found to be closed and locked. The soil stockpile was stabilized with well-

-lined and sealed secondary containment. There is no identification posted on the tanks. There are concrete barriers protecting the containment from vehicles. The e-stops are not yet installed outside the barriers. The tanks are grounded, and have static dissipaters. There is no truck grounding area for trucks collecting condensate and/or produced water yet installed. There is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.
