0003314919 2018-03-30 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams & Austin Gutridge, I witnessed that Conserv Incorporated was tripping 2.375 inch diameter tubing in at 2100 feet planning to set it at 6240 feet. Due to the amount of rain the circulating pit was full and no available vac trucks plugging will not re-commence until Monday.
0021707203 2018-04-12 Plug / Plug Back No Violations Conserv Incorporated rigged up the wireline truck and tripped within the 9.625 inch diameter casing with a gauge tool to evaluate the casing to see if the cast iron bridge plug could make it to the desired depth of 3160 feet. The gauge tool sat down at 2167 feet and could not descend downhole any further. The gauge tool was removed from hole and a cement bond logging tool was tripped back in hole. The tool sat down at 2167 feet and the hole was logged from there up to evaluate the cement on the backside of the 9.625 inch diameter casing. The bond log showed a cement bond from 2167 feet to surface. The decision was made to not use the cast iron bridge plug since it could not get down to 3160 feet. 2.375 inch diameter tubing was tripped within the 9.625 inch diameter casing to 3160 feet.
0033577630 2018-09-14 Plug / Plug Back No Violations The inspection is conducted as supplemental information for the plugging operations. I met with Landowner Phil Dobeck on his water well. The water well had coordinates of 39.91759, --foot difference. The plates were cut and got a measurement of 9 5/8-inch casing to 13 3/8-inch casing on the cement. It was 3 feet down. The 13 3/8-inch casing and 20-inch casing was one foot down. The measurement on the annulus of the 20 inches and 26-inch casing was down 22 inches.

0076016295 2018-04-04 Plug / Plug Back No Violations Conserv Incorporated tripped within the 5.5 inch diameter casing with a sinker bar and collar locator to try and verify if the casing was parted. They tagged up at 4155 feet where the casing was shot. EQT believes that the casing was parted at that depth. Energy Fishing and Rental Services arrived on location with casing jacks. The casing jacks lifted the casing enough to remove the slips and the packing rom the casing head. The 5.5 inch diameter casing was not completely free. Conserv Incorporated tripped inside the 5.5 inch diameter casing and shot the casing with a casing cutter at 4101 feet. The shot was unsuccessful and 5.5 inch casing was not free.

The location was gated and grating was protecting the cellar with proper identification present at the entry and at the wellhead. Fluid present in the cellar was field tested revealing a pH of 7 and chlorides of < 33 parts per million (ppm). 6 inches of fluid was present in the tank containment and was also field tested revealing tested revealing a pH of 7 and chlorides of < 33 ppm. Upon leaving the location I met with the land owner Mr. Dobeck who stated he has had his mineral released.

Job circulated good.
0639768308 2019-02-06 Plug / Plug Back No Violations The inspection revealed that Kelly--cone. The camera being ran was owned by Marshall/ Miller & Associates. The camera was rigged up and ran into the well bore. It set down at 140 feet with an obstruction. There were no signs of bubbling on the camera run. The rig hooked up flow line and was attempting to blow air down 4.5 inch open ended drill pipe to dry hole up. The camera was rigged back up and ran to a depth of 152.8 feet with a 13- foot Kelly- bushing depth. There was no bubbling present at this depth.

0792636353 2019-03-07 Plug / Plug Back No Violations The inspection revealed that Fountain rig 8 (Schramm 250) was breaking down the poor boy flow tee and looked down into the 20-inch riser and noted no bubbling present. EQT made decision to have Rig 8 rig up the 18 11/16-inch burn over shoe and trip into the 20-inch casing and tag the cement. They tagged at 105 feet. The rig is cleaning up the hole and will trip out of the well and get ready to rig the Black Hawk tools to run in with a Weatherford 20-inch cement retainer. ConServ well services pumped on the retainer and set the packer element at 3000 psi. The rig is going to sting into the retainer and place 300 psi on the top of the cement plug below the retainer and sting out. ConServ well services could not get the pressure to build with no circulation after stinging into the retainer. Once the tool is unstung then circulation was established. The decision was made to place collars on the new stinger and then place thru the retainer and try to pressure up in the morning. If not able to then pressure up, then squeeze cement there and shut down.

I met with the land owner Mr. Dobeck who stated he has met with Pin Oak representatives and they are in talks.

Phil Dobeck the landowner called on 10-17-18 and stated that there were bubbles coming out of the casing down in the cellar. Brent Bear and I went to the well and verified the bubbles were coming from the 26 in casing. The cellar contains over 1 foot of water in the bottom and the casing is full on top of the cement. I stuck a probe into the cement and found that it was hard but apparently has channeled through the cement.

All the casing is within the 20-inch diameter casing. Haliburton is on site rigging up and will be placing gas block cement back to surface. They will be running HalCem system of 370 sacks of cement at 1.229 yield and 15.6 lb./gallon. The job was pumped, and the rig cleaned out all the drill pipe. They then were disassembling the flow stack. There was 10.5 barrels of cement pumped to surface of returns.

Marshal Miller and Associates are on site to run the camera in the well bore and look for gas bubbling. There is two 1170 air packages and a booster on site. The rig was blowing down the well thru the 4.5-inch drill pipe. The camera was running in the well bore and could only get to the depth of 183 feet on the second run.

During our inspection the well could be seen bubbling within the cellar from the backside of the 20-inch diameter surface casing. EQT bled down the gas from the 9.625 by 13.375 inch diameter annulas prior to arrival. Fountain Qual Rig 8 was in the process of rigging down and skidding over to 34059242850100.
1522253562 2018-03-28 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams, we met with EQT Plugging Specialist Josh Williams and reviewed the work done yesterday witnessed by ODNR Inspector Justin Smith. Conserv Incorporated (CI) had removed the check valve, safely bled well down, set an Owen Oil Tools cast iron bridge plug at 6248 feet and successfully performed a 1500 pound per square inch pressure test for 20 minutes. Conserv Incorporated Wireline Service (CIWS) was rigged up and performed a cement bond log (CBL) on the 5.5 inch diameter production casing revealing top of cement was at 4760 feet. CIWS rigged down and moved to the 34059242850100 Dobeck 580014 to perform a CBL on the 5.5 inch diameter production casing.
1522682083 2018-04-02 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed that Conserv Incorporated (CI) had tripped in 2.375 inch diameter tubing to the depth 6249 feet. CI gained circulation with 10 barrels of fresh water and pumped a 500 foot 53 sack Class A bottom hole plug from 6249 feet to 5749 feet. Cement was displaced with 22.25 barrels of fresh water. CI tripped tubing out to 5649 feet and pumped 9.5 barrels of 6% gel from 5649 feet to 5225 feet. CI tripped tubing out to 5243 feet to and pumped a 350 feet 37 sack Class A Clinton plug from 5243 feet to 4893 feet. Cement was displaced with 18.9 barrels of fresh water. CI tripped tubing out and will tag plug along with shooting 5.5 inch diameter casing off tomorrow.

1523886349 2018-04-13 Plug / Plug Back No Violations Conserv Incorporated had 2.375 inch tubing at 3160 feet within the 9.625 inch casing. The hole was circulated and a 550 foot plug was pumped using class A cement from 2160 feet to 3160 feet. A 6 percent gel spacer was set from 2160 feet to 1750 feet. The tubing was pulled up to 1750 feet and a 37.9 barrel class A cement plug was pumped from 1750 feet to 1250 feet.
1533559573 2018-08-01 Plug / Plug Back No Violations During my inspection Fountain Qual Rig 8 began to skid over from 34059242850100 to prepare to drill out the surface plug. The 3-inch diameter valve on the 13.375-inch diameter intermediate casing had a valve present reading 60 pounds per square inch. When that valve was opened the rigs gas monitor alarms went off recording 19% lower explosive limit. Indicating that there was in fact gas coming from the back side of the 9.625-inch diameter second intermediate casing.

BJ Services rigged up on location a pumped a cement plug from 1583 feet to 1283 feet a crossed the milled section. The plug consisted of 109 sacks of class A Gas Block cement. The plug was pumped through 4.5-inch diameter drill pipe weighing 16.6 pound per foot.

Fountain Quail rig 8 was over well and was placing a composite plug from Haliburton at a depth of 609 feet inside the 9 5/8--inch casing and remove the tool and trip open ended drill pipe back in to depth of to set the 150 foot of cement plug. The cement being ran by B J Services is a gas block style cement with 56 sacks of 1.19 yield and 5.24 gallon per sack water requirement. 609-459 foot is plug interval. Good circulation was established. A sample of the slurry from the tub of the pump truck was obtained.

Fountain Quail rig 8 was over well and Baker Tool Company was onsite rigging up a mill over tool. The mill over tool would be over the 9 5/8-- over pipe and a scallop on the bottom of the tool. There may a problem with the threads on the wash-over pipe. The pipe will not screw together.

The decision was made for KLX Tool company to build an 18 5/8-inch burn over shoe with tungsten carbide welded to the casing. The piece on the retainer is cast iron and will mill up fast according to Black Hawk Tool company. Then burn down over the top set of slips. The tool should pull out. The EQT will have Fountain Quail cleanup hole with finish mill in the 20-inch casing.

KLX tool company has an 18 11/16--inch cement retainer from Black Hawk tool company. The burn over shoe from KLX was pulled out and the magnet was ran in to retrieve all metals. Most of the slips were retrieved back and the top ring was milled completely up. The magnet was trip out and was removed, and the rig rigged up the center spear and tripped in to the hole. The Black Hawk Cement retainer was tripped out of the hole. KLX hand noticed that the lower slips were at the 51 feet mark. KLX will bring a larger magnet to retrieve out all pieces tomorrow.

The flowline to the shaker was frozen up and would have to be chipped out.

From 1800-2400 psi is the pump pressure to set the tool.

1617822501 2018-03-05 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Ryan Armstrong, I witnessed the well was equipped with a Cactus casing wellhead with valves in the closed position with no flowline installed. Production equipment included a sand trap, a GP Unit, a sand purge tank, 8-400 barrel tanks, 140 barrel tank and a 100 barrel purge tank all within containment and a gas combustor and will be shared with the 34059242850100 Dobeck 580014. The location was gated and grating was protecting the cellar with proper identification present at the entry and at the wellhead. A plug permit was issued on 1/25/18. I met with land owner Phil Dobeck and discussed the upcoming plugging of the wells. A call was placed to EQT Supervisor Rick Green and requested an update on when plugging will commence. Mr. Green returned my call and stated that a rig should arrive later this week and deconstruction of the production equipment should begin later today.

-55 feet. This is where the 20-inch cement retainer from Black Hawk got hung up and partially set. The decision was made to run in with 18 5/8 burn over shoe. The shoe gauges to 18 11/16 inch. The rig ran the tool to 190 feet. The rig soap swept the hole and was rigging up the camera for the second time. The camera was ran in the well and rotated as going down and caught a collar at 35.8 feet and verified no bubbles. Then ran into a depth of 77.8 feet at 2nd joint and verified no bubbles. Then ran to a depth of 116.7 feet at 3rd joint and no bubbling, then ran to a depth of 159 feet at 4th joint and bubbling in 4 spots at collar. The pipe could be two different weights of pipe. The depth of 175 feet revealed a cement sheath that was cleaned away from the pipe. The depth of fluid is at 183.3 and there was bubbling at the top of the fluid level.

The Fountain Quail rig is currently over the well to monitor gas leaks. The inspection revealed the well could be seen bubbling within the cellar from the backside of the 20-inch diameter surface casing. EQT bled down the gas from the 9.625 by 13.375-inch diameter annulus prior to arrival. Fountain Qual Rig 8 was in the process of rigging down and skidding over to 34059242850100.

1956462104 2018-04-03 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Ryan Armstrong, I witnessed Conserv Incorporated (CI) nipple down the casing wellhead. Conserv Incorporated Wireline Service (CIWS) rigged up and prepared a 4.5 inch diameter casing cutter. CIWS tripped in and finding fluid level at 411 feet, tagged the Clinton plug set 4/2/18 at 4818 feet and shot the 5.5 inch diameter casing off at 4465 feet. CIWS tripped out of the casing and EQT welding contractor welded the slips to the casing. CI speared the casing and made multiple unsuccessful attempts at freeing the casing pulling 57% over string weight at 140000 pounds. Cement Bond Log was reviewed again and decision was made to cut casing again. CIWS tripped in and finding fluid level at 388 feet and shot the 5.5 inch diameter casing off at 4155 feet. CIWS tripped out of the casing and CI speared the casing and made multiple unsuccessful attempts at freeing the casing pulling 70% over string weight at 140000 pounds. Operations shut down for the day to review the data.

The plug remained at surface. EQT Supervisor Rick Green stated that they would be placing a steel plate on top of the well with proper identification.
