0228131856 2019-03-21 Plug / Plug Back No Violations The Fountain Quail rig 8 (Schramm 250) had rigged up Keystone Energy wireline services and ran a camera in to the top of the Baker cement retainer at 195 feet. The camera revealed no bubbling at the top of the 9 5/8-inch cement retainer and pulled back to 180 and there was bubbling from the gas. Universal well services were in to set a gas block cement plug from 195-120 feet with 30 sacks of Class A cement + 5% salt+ .75%CFL-
0289319637 2017-03-22 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed the well the well was equipped with a Cactus casing wellhead with valves in the closed position with no flowline installed. Production equipment included a sand trap, a GP Unit, a sand purge tank, 8-400 barrel tanks, 140 barrel tank and a 100 barrel purge tank all within containment and a gas combustor and will be shared with the 34059242800100 Dobeck 580015. The location was gated and grating was protecting the cellar with proper identification present at the entry and at the wellhead. Fluid present in the cellar was field tested revealing a pH of 7 and chlorides of < 33 parts per million (ppm). 6 inches of fluid was present in the tank containment and was also field tested revealing tested revealing a pH of 7 and chlorides of < 33 ppm. Upon leaving the location I met with the land owner Mr. Dobeck who stated he has had his mineral released.
0304923872 2018-05-31 Final Restoration No Violations The purpose of my inspection was a status check. During my inspection the cellar was 2 feet from being full of rain water and bubbles could be seen coming to surface of the water. I spoke with Rick Green with EQT and asked that he meet me on location with a water truck and confirm where the bubbling was coming from. While utilizing my Ventis Pro 4 gas detector 0 LEL was recorded and no smells of gas were present. Mr. Green agreed to meet me at the wellsite on 6/4/18.

The camera was tripped within the 9.625-inch diameter casing and did not see any bubbling or gasing within the casing. 0343565202 2018-03-19 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams, I witnessed the well was equipped with a Cactus casing wellhead with valves in the closed position. All production equipment has been disassembled and removed from location. Conserv Incorporated Rig #6 was onsite with plans to rig up and commence plugging of the well later this week.
0368071576 2018-04-18 Plug / Plug Back No Violations On 3/27/18 Conserv Incorporated set a cast iron bridge plug at 6253 feet within the 5.5 inch diameter production casing. During my inspection today Conserv tripped 2.375 inch diameter tubing down to 6250 feet and set a 500 foot plug from 6250 feet to 5750 feet. The plug consisted of 11.1 barrels of class A cement. A 6 percent gel spacer was pumped and the tubing was pulled up to 5225 feet. A class A cement plug was pumped from 4900 feet to 5225 feet. A gel spacer was pumped and a the tubing was pulled up to 3800 feet where a 300 foot class A cement plug was set from 3500 feet to 3800 feet.
0421390854 2018-05-01 Plug / Plug Back No Violations Conserv tripped in hole with the 2.375 inch diameter tubing and tagged the plug set 4/30/18 from 1750 feet to 1250 feet. The plug was tagged at 1304 feet. Some of the cement was squeezed through the perforations to fill the void on the backside of the 9.625 inch casing. A 6 percent gel spacer was pumped from 1304 to 500 feet. The 2.375 inch tubing was pulled up to 500 feet and a class A cement plug was pumped from 500 feet to surface.

Quail Drilling rig #8 is on site and KLX is on site to cut the 9 5/8-- circulation. The rig broke circulation and started to cut the pipe. The well quit circulating. The pipe was cut at this point and retrieved the tool out of the well. The casing crew was on the way to trip the casing out of the well.
0623219531 2018-08-02 Plug / Plug Back No Violations On 8/1/18 the well was shut in overnight to see if gas would accumulate. During my inspection the Fountain Qual Rig 8 removed the Kelly off the drill pipe and checked the drill pipe for gas readings. No readings were recorded. A night cap was installed with a pressure gauge. The rig crew continued to work on skidding the rig to 3405924280. The cellar was power washed and water was removed with super sucker water truck.

The location was gated and grating was protecting the cellar with proper identification present at the entry and at the wellhead. On July 27, 2017 I received a call from Mark Van Tyne of Pin Oak Energy who stated that Pin Oak may be acquiring the Dobeck 580015 & Dobeck 580014 from EQT. I met with the land owner Mr. Dobeck who stated he has met with Pin Oak representatives and they are in talks.
0650839104 2018-06-04 Plug / Plug Back Violations Noted The purpose of my inspection was to determine why the rain water within the cellar was rapidly bubbling. Plugging operations for the well were completed on 5/1/2018. During my inspection, I was accompanied by EQT company man Rick Green. Mr. Green had a water truck arrive on location to remove the water from the cellar. The water was removed down to the top of the 9.625 inch diameter intermediate casing which appeared to be the source of the bubbling. The steel plate was removed from the top of the casing and gas appeared to be bubbling up through the water within the casing. Mr. Green held his gas monitor over the 9.625 inch casing and recorded an LEL of 15. A plumb bob was lowered into the 9.625 inch casing and the top of the cement surface plug was tagged at 16 feet from surface. I asked Mr. Green to have EQT present a plan to ODNR for approval or denial that would eliminate the flow of gas to surface. EQT shall correct the failed plugs and prevent the escape of natural gas into the atmosphere.

-inch stainless steel ball was pumped by Conserv Service company pump truck. The tool was set, then the string was pulled out of the hole. Conserv will pump cement from top down with gas block cement. 140-0 with 106 sacks of Class A cement with gas block additive which consisted of .1 gallon per sack of c-24l, .2 gallon per sack of c-17l, .05 gallon per sack of c-37l and .05 gallon per sack of c-41l. two samples were obtained of cement slurry and was weighed with scales to 15.6 lb. per gallon. The fresh water had a PH OF 6 and a temperature of 49 degrees.

During my inspection I spoke with EQT consultant Dave Hancock and he stated that on 7/30/18 the hole was blown down to 770 feet and Cardo Services rigged up with their camera and tripped in the 9.625-inch diameter casing to verify where the gas was leaking into the wellbore and no bubbling was detected. A copy of the camera run was reviewed. EQT made the decision to blow the hole down deeper to prepare to trip back in with the camera on 8/1/18.

They are getting rigged up and will start drilling this afternoon.

Quail Drilling rig #8 is on site and Haliburton is on site to set a Composite bridge plug at 553 feet. The tool is on the bottom of 13 joints of 4.5-inch drill pipe at 16.60 lb. per foot. B J services is on site with cement equipment and set the composite tool and placed a 200-foot cement plug with 73 sacks of regular class A cement with Gas Block cement on the composite. B J set the composite by pumping on the tool and the rig pulled all pipe out. The rig ran in with open ended 4.5-inch drill pipe and set the cement plug on top of the composite plug. The rig then pulled out 4.5 inch drill pipe and shut down.
0881274820 2019-03-15 Plug / Plug Back No Violations Fountain Quail rig 8 (Schramm 250) has drilled to 180 feet with a 12 3/8 tri-cone bit inside the 13 3/8--cone bit and ran inside the 9 5/8-inch casing from 180-225. The rig tripped out of the well and ran a 12 3/8-inch mill in to clean up the inside of the 13 3/8-inch casing. The rig made several soap sweeps to clean up the hole. Marshall, Miller & Associates ran a camera into the well to a depth of 227 feet. The video showed the 9 5/8 casing was to one side of the 13 3/8-inch casing. The back side of the 9 5/8-inch casing was gassing at 181 feet and the inside of the 9 5/8-inch casing was gassing at 227.

1157168480 2018-04-26 Plug / Plug Back No Violations Conserv pumped a class A cement plug from 2504 feet 2804 feet a crossed the 5.5 inch casing cut. The plug was set 52 feet within the 5.5 inch diameter casing and 248 feet within the 9.625 inch diameter casing. The tubing was removed from hole. Conserv Wireline tripped in with a gauge tool to determine if the cast iron bridge plug would be able to get down to roughly 2450 feet. The gauge tool successfully made it down to the desired depth and it was tripped out of hole. The cast iron bridge plug was rigged up to wireline and ran down to 2450 feet and was set successfully within the 9.625 inch diameter casing. Wireline was tripped out of hole and tripped back in and set 10 perforations within the 9.625 inch diameter casing from 2280 feet to 2290 feet and then 10 more shots from 2150 feet to 2160 feet.
1168435986 2018-10-18 Plug / Plug Back No Violations The inspection is conducted as a follow-up to the plugging operations that were completed. Brent Bear and I were on site looking at the plugged well. There is a no bubbling on the large flange casing spool. The cellar has water on the floor. The cement has been verified at the top that there is no bubbling. The large spool will have to be removed.

During my inspection a bond log was ran to verify cement bond on the backside of the 9.625 inch diameter casing from 2650 feet to surface. The bond log was reviewed and EQT decided to perforate the areas with the weakest bond. Conserv tripped back within the 9.625 inch casing with 10 shots and perforated the casing from 2600 feet to 2590 feet. Wireline was tripped out of hole and the 2.375 inch diameter tubing was tripped back in hole to 2804 feet.

The location was gated and grating was protecting the cellar with proper identification present at the entry and at the wellhead. . A plug permit was issued on 1/25/18. I met with land owner Phil Dobeck and discussed the upcoming plugging of the wells. A call was placed to EQT Supervisor Rick Green and requested an update on when plugging will commence. Mr. Green returned my call and stated that a rig should arrive later this week and deconstruction of the production equipment should begin later today.

Conserv pulled up on the 5.5 inch casing and the casing was free. 2752 feet of the 5.5 inch diameter production casing was removed from hole.
1525267069 2018-04-30 Plug / Plug Back No Violations Conserv Incorporated tripped within the 9.625 inch casing with the wireline perforating gun and 10 shots spread out in 10 feet. The casing was perforated at 1710 feet to 1700 feet. The wireline was tripped from hole and 10 more shots were wired up. The wireline tripped back in the 9.625 inch casing and shot 10 shots from 1660 feet to 1650 feet. Conserv tripped the 2.375 inch tubing back in hole and set a 500 foot plug a crossed the perforations 1750 feet to 1250 feet. The plug consisted of 37.9 barrels of class A cement.
1531324568 2018-07-11 Plug / Plug Back No Violations The purpose of my inspection was a follow up on compliance notice -650839104. The compliance notice was a result of a failed plug job that left the well leaking gas within the 9.625--plug the well. A move in date has not yet been given. I contacted landowner Phil Dobeck to make him aware that EQT plans and is required to move back in to re-plug the well. During my inspection today the well remains to gas and EQT has not moved in at this time.

During my inspection Fountain Qual top hole rig number 8 was rigged up on location preparing to drill out the cement plugs. The rig was being service and not in operation. A walk around was completed with David Hancock, a consultant for EQT. Mr. Hancock stated that the would like to be started drilling out the cement plugs by this afternoon.
1533136128 2018-07-30 Plug / Plug Back No Violations The purpose of my inspection was a status check on remediating a leaking plugged well. During my inspection the Fountain Quail Rig 8 was in the process of rigging up an air compressor to prepare to blow the hole down to remove fresh water from within the 9.625-inch diameter casing to remove some hydrostatic pressure. The well was not leaking at that time, so they attempted to remove hydrostatic pressure to encourage the well to leak to verify the problem area.

1534858304 2018-08-20 Plug / Plug Back No Violations Fountain Qual Rig 8 was rigged up on location with 4.5-inch diameter drill pipe weighing 16.6 pounds per foot at 1343 feet in depth within the 9.625-inch diameter second intermediate casing. BJ Services rigged up on location with a pump truck and bulk cement truck. A pre-job safety meeting was held with all personnel on location. BJ pumped 375 barrels of 6 percent gel at 9 pounds per gallon and the rig pulled up 500 feet. BJ pumped an additional 125 barrels of gel and the rig tripped back into 1350 feet and BJ pumped 125 barrels of gel and 15.25 barrels of Class A cement weighing 15.6 pound per gallon and displaced with 16.2 barrels. The drill pipe was tripped up out of the cement.
1535461577 2018-08-22 Plug / Plug Back No Violations During my inspection I attended the pre-job safety meeting with ODNR Pat Shreve, EQT, Fountain Qual Rig Hands, Halliburton, and BJ Services. Following the meeting Halliburton rigged up their composite bridge plug to the drill pipe. Fountain Qual tripped the drill pipe into 551 feet in depth within the 9.625-inch diameter intermediate casing. BJ Service rigged the pump truck up to the drill pipe and with the assistance of the Halliburton tool hands the composite drill plug was set at 551 feet. The drill pipe was tripped out of hole and the setting tool was removed. The drill pipe was tripped back in hole open ended. BJ Services hooked up to the drill pipe and pumped a 73-sack class A Gas Block cement plug from 551 feet to 351 feet within the 9.625-inch diameter casing.

EQT Production Company has submitted a plan to the division to reopen to plug and abandon this well.

Then the camera was running in the well from Keystone wireline services and showed the Baker tools can be set with out getting tore up. Baker tool set an easy drill cement retainer in the 9 5/8-inch casing at 195 feet.

The location was gated and grating was protecting the cellar with proper identification present at the entry and at the wellhead. The water impoundment was securely fenced in with a life preserver at the locked gate.

The casing jacks were rigged up and pulled 180,000 pounds to pull the 5.5 inch diameter casing out of the casing head. The casing head was disassembled and the backside of the 9.625 inch diameter intermediate casing had a small amount of gas that was bled down immediately. After bleeding down the gas it did not reappear. Conserv Incorporated pulled stretch on the casing while running within the 5.5 inch diameter casing with the wireline free point locator. The tool was calibrated and then was ran down to tag the prior cement plug top at 3500 feet. The tool verified a free point at 2,777 feet. The free point locater was tripped out of hole. Conserv tripped in hole with a casing cutter and cut the casing at 2751 feet.

Conserv Incorporated (CI) had removed the check valve, safely bled well down, set at cast iron bridge plug at 6249 feet and successfully performed a 1500 pound per square inch pressure test for 20 minutes. Conserv Incorporated Wireline Service (CIWS) was rigged up and performed a cement bond log (CBL) on the 5.5 inch diameter production casing.
