0016829936 2015-04-01 Production Wells No Violations On the day of my inspection accompanied by Southeast Regional Supervisor Brent Bear, we with Eclipse Facilities Superintendent Rick Matheson and Eclipse Facilities Foreman Greg Offenberger and completed site inspection of the Hayes Central Gathering Facility. Mr. Matheson explained in detail flow of fluids through the facility and at our request measured set back distance of the flare stack to the stock tanks which met the States requirement according to OAC 1501:9-9-05 (B). I stated my concerns with the size of the flare and wind conditions and Mr. Matheson stated that once they got their additional air permit from the EPA they would be able to eliminate the flare with running of the second compressor on site. I placed phone call to EHS Supervisor Bruce Carpenter stating my concerns with the flare and safety of traffic and personnel on the site and Mr. Carpenter stated that they were awaiting the air permit from the EPA and he would let EHS Vice President Melissa Hamsher know the concerns that I stated.
0202509370 2017-10-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a plunger lift system with valves in the open position. The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 405 pounds per square inch on the tubing and 470 pounds per square inch on the casing with annular pressure monitoring in place. Proper identification and production records were current. The Above Ground Storage Tank (AST) remains onsite along with 2-500 barrel empty frac tanks within containment.
0547271060 2014-12-11 Production Wells No Violations On the day of my inspection I received a phone call from Eclipse EHS Field Representative Ken Durst about a spill on the Hayes Central Gathering Facility. A plug was missing in a coupling on a transfer line to the transfer pumps which are currently under construction and as the levels in the condensate tanks filled gravity forced fluid down the transfer line and out of the coupling. Approximately 5-10bbls leaked on to the pad to an area of 10 feet by 80 feet. Upon arrival I found that clean-up was taking place with a backhoe, skid steer and shovels. Placing contaminated stone onto containment and awaiting roll off boxes for proper disposal. 4 wells from the Hayes Pad, 3 wells from the Fritz Pad and 4 wells from the Pora Pad currently feed into the CGF. Returning to the CGF I found that clean-up was completed and done satisfactory.

The production equipment included a sand trap; a GP unit and 2-400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 700psi on the tubing and 970psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. We completed a walk around with Eclipse Well Tender Bradley Paisley.

Gauges present read 1110psi on the tubing and 1340psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. Discussion was held on the readings with Eclipse Lease Operators Bradley Paisley and Danny Hreha.

Gauges present read 350 pounds per square inch on the tubing and 415 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current. An Above Ground Storage Tank (AST) was present on the northwest corner of the well pad supplying fresh water to the Frank Miller pad for frac with support being provided by Great Lakes. Water was field tested revealing chlorides of <33 parts per million, pH 7 and 60 degrees.
1393519886 2014-02-24 Surface Facility Construction Violations Noted On the day of my site inspection, I witnessed a slip that occurred on a hill at the west side of the location after the excavation of the water impoundment had started, the area disturbed is approximately (150 x 60) I placed a phone call to Eclipse Resources to inform them of the slip and to see if a member of there staff could meet with me on the location to discuss a restoration plan for the disturbed area. Trumble Excavating was on site that day and started making temporary repairs until an engineered plan was in place, some of the disturbed area was outside of the L.O.D. Eclipse Resources stated to me that they purchased the disturbed area outside of the L.O.D. from the landowner after the slip had occurred. Engineers from Eclipse Resources was on location the next day with an engineered plan to restore the disturbed area. The requirements to meet The Division of Oil and Gas standards will be to construct disturbed area to prevent any further slip.

1742826865 2018-06-11 Production Wells No Violations The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 480 pounds per square inch, 624 pounds per square inch on the production casing, 150 pounds per square inch on the 9.625 inch diameter intermediate casing, and 0 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of two 400 barrel tanks, six sand traps, six gas production units, and one indirect line heater. An above ground storage tank was situated in the Northwest corner of the pad. The identification was located at the wellhead.

2055562181 2016-03-14 Production Wells No Violations On the day of my inspection following up from the incident Friday 3/11/16 at the Hayes CGF which is owned and operated by E2, I met with E2 Production Specialist Chap Barker, Ohio State Fire Marshall Michael Stellfox, Quaker City Fire Captain Todd Eagon and Fireman Dave Shackel. We completed a walk around of the gathering facility and reviewed the components of the trailer along with the facility. The trailer contains a flushing unit which was being used to flush out the three oil stabilizer units in which two of the units were successfully treated. Upon treating the third unit the trailer became engulfed in flames and personnel activated the emergency shut down control and exited the facility. Local fire departments were dispatched to the facility and extinguished the fire. The trailer was a total loss with only a gauge being damaged and smoke damage to the stabilizer unit. E2 will be onsite tomorrow 3/15/16 to disassemble the flushing unit to determine the source of ignition.
