0109235313 2014-06-23 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Universal Well Service break circulation with fresh water then displace 440 sacks of Type-1 cement to a depth of one hundred foot with twenty barrel of cement returns into the cellar.
0161797931 2015-04-16 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to ORC 1509.072A.
0278050287 2014-09-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that clean-up from Sunday nights spill was completed and done satisfactorily. Patterson UTI Drilling Rig #328 had successfully ran and cemented their production string and were setting their slips and preparing to skid to the Weekender 4H. Completed walk around with Company Man Billy Shafer and pointed out the rig containment had two small tears. Billy Shafer stated that a containment crew will be on site after rig skid is completed and repairs will be made.

Production equipment included a sand trap, a line heater, a GP Unit along with 2-400bbl purge tanks within containment. The well was equipped with Sweco wellheads and production trees with valves in the closed position with 2900psi on the tubing and casing. The well will go to production tomorrow with all fluids and gas going to the Borton CGF. Completed walk around with Consultant Cristo Giannareas revealing that secondary containment and proper identification was in place and preparations were being made to flowback the 4H.
0677815932 2014-09-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that the rig has been cleaned and the majority of the stone removed and replaced. Contaminated vegetation removal is underway and should be completed by this evening. I made request to Company Man Billy Shaffer to begin cleaning the top of the two trailers that got hit by mist and then finish the removal of contaminated stone.
1105390596 2017-06-07 Production Wells No Violations On the day of my inspection I witnessed production equipment included a sand trap, a line heater, a GP Unit, a blow case skid and 2-400 barrel purge tanks within containment. The well was equipped with a plunger lift system with valves in the open position with 420 pounds per square inch on the tubing and 625 pounds per square inch on the casing. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF.
1202560795 2016-06-13 Production Wells No Violations On the day of my inspection I witnessed production equipment included a sand trap, a line heater, a GP Unit along with 2-400bbl purge tanks within containment. Upon arrival I met with Company Man Sam Bond-Zielinski and Eclipse EHS Representative Mike Hartshorn completing walk around I witnessed that 5.5bbls of oil based mud had sprayed the side of the doghouse, derrick and misted across the south side of the well pad. The light mist affected 40 foot by 50 foot area of vegetation outside well pad containment and a 40 foot by 50 foot outside the drilling rig primary containment. Grant Smith, PETA and the Patterson Drilling Rig #328 crew were on site removing contaminated stone and power washing the drilling rig with a vacuum truck on site pulling up contaminated water.
