0056984811 2012-09-10 Drill / Deepen / Reopen No Violations I am on site today to plug back another well on this pad. While I was waiting on cement trucks the Rat Hole Drilling crew began drilling this well. I feel this well will collapse and should not be drilled with an Auger type rig. Later I received a call from Inspector Grimm who is covering the plug back and he advised that this well had collapsed and will need to be plugged back. This well did collapse to the point that there was no hole to plug back. Inspector Grimm advised them to bring in a deep reaching Backhoe and dig down and then back fill with rock and stone.

1348658251 2012-09-16 Drill / Deepen / Reopen No Violations Arrived on location and found that while conductor hole was being drilled it started to cave-in between the kinsey 1 and the kinsey 2. Company wasnt sure what to do and wanted my recommendation on how to proceed. Going to plug both holes and remove caved-in area and replace with stone.
1610940420 2012-09-10 Drill / Deepen / Reopen No Violations Arrived on location by request of operator because the conductor hole collapsed while drilling. Hole is 36 in diameter and was 47 deep. Well caved in at 37 and crew cannot keep it open . Company wants to plug it back to surface with a ready mix truck.
1670384560 2016-03-29 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

9 400 bbl. Production tanks 2 Producing wells 2 Sand separators

1 Low Pressure Separator 1 Combustor 1 Line Heater 1 Compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.
