0523238190 2012-09-10 Drill / Deepen / Reopen No Violations I am on site today to witness the plug back of the Conductor portion of this well. The Operator contacted me at 1:00 am to inform me that the hole had collapsed. I advised that the well could be plugged back using 9 sack grout. The well is scheduled to be plugged back at 11:00 AM . I have contacted Inspector Grimm to cover the plug back while I am on another job. Before I left the Rat Hole drilling operator informed me he is to start the next well while waiting for the cement trucks. I ask if he should begin because the two wells could communicate. He said that the company understood the risks and advised he continue drilling to see if the next one would have the same problem.
1507835210 2017-10-12 Production Wells No Violations On the day of my inspection I found the pad to contain two producing wells. Production equipment includes 9 400 barrel tanks and 2 gpus. No api numbers on the actual wellhead and no pressure gauges for surface or intermediate casing noted. Will contact Chevron to correct these issues.
1710878187 2016-03-29 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

9 400 bbl. Production tanks 2 Producing wells 2 Sand separators

1 Low Pressure Separator 1 Combustor 1 Line Heater 1 Compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.
