0019283136 2013-03-07 Drill / Deepen / Reopen No Violations Stringray Cementing cemented 115 of 26 casing with 250Sacks of Class A cement. See the Casing Ticket for more information.
0578940290 2018-10-01 Production Wells No Violations On the day of my inspection I am on location to follow-up ona phone call from Brad Burwell yesterday. While pulling out the original earthen tank containment Gulfport discovered some condensate and gotten underneath the containment through the years. Brad stated that the tanks do not have any signs of leakage. 17 roll off containers (40 cubic yards each) of material were removed from site. Brad said that Hall and associates were onsite to verify that all condensate had been removed. Largest excavation was a 15 feet by 15 feet by 8 feet deep. During my insepction I witnessed this area had been backfilled and graded. Operator stated they placed clean backfill material in one foot loose lifts and compacted it. A berm was not installed in this area after backfill and I mentioned to Brad that it needs installed to match the rest of the pad berm. Brad sent over emails and photographs. I forwarded them to engineering staff Paul Logan and Jerry Smith. This well has 300 surface annular pressure and 0 on the intermediate. Prodcution equipment includes four vertical seperators, four sand traps, four GPUs, one combustor, one 50 barrel steel brine tank in its own earthen containment, as well as six 400 barrel steel tanks that have been removed from containment.
0985016475 2013-03-29 Drill / Deepen / Reopen No Violations On the day of my inspection I picked up a cement ticket from previously witnessed 18.625 casing job. Mark Moreland was present to witness 13.375 cement job. No violations noted.
1168181237 2013-10-15 Preliminary Restoration No Violations The site passed preliminary restoration according to 1509.072A.
1364308472 2013-03-26 Drill / Deepen / Reopen No Violations I am on site today to witness the running and cementing of the 13-3/8 inch casing string. Yost Drilling Rig # 7 has drilled a 17.5inch diameter hole to 1762 ft. and set 1722 ft of casing. Stingray Cementing service c/w 1390 sks of class A 15.6 lb cement. I witnessed 25 bbls of good cement returns to surface.
1364309314 2013-03-22 Drill / Deepen / Reopen No Violations On the day of my inspection I covered a surface job. 18.625 casing was ran and cemented with good cement returns. See cement ticket for details. No violations noted.
1366824737 2013-04-24 Plug / Plug Back No Violations I am on site today to plug the 12-1/4 inch diameter section of the well back. Schlumberger Cementing is plugging back today from 6977 ft in 500 ft stages. 6977 ft to 6477 ft pull up 500 ft. 2 nd plug from 5977 ft to 5477 ft then pull up again 500 ft. 3 rd plug from 4977 ft to 4477 ft pull up 500 ft. Set 4 th plug from 3700 ft to 3277 ft. Shut in for 24 hours. Then Sidewinder M- 60 will re-drill this same section and try to keep from going outside the Hardline or drill corridor.
2032651842 2016-03-15 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of a plunger lift system. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

4 Line heaters 1 Combustor 1 Low-pressure Separator 1 50 bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A.
