1070921524 2014-08-19 Drill / Deepen / Reopen No Violations On the date of inspection I witnessed that the Patterson 261 rig had drilled to a depth of 828 feet and was currently broke down awaiting parts.
1104893587 2015-05-28 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
1408399471 2014-08-18 Casing String Witnessed No Violations Patterson 261 has drilled a 17.5 open hole to a depth of 595. The 20 conductor was set to 120. 583 of 13.375 54.5#/ft casing was ran. Otex is onsite to cement the casing back to surface using 540 sacks of class H (15.6 ppg; 1.21 yield) cement at 50% excess. Displacement calculated to be 83.8 bbl leaving a 44 shoe. Estimated 23 bbl of cement returned to surface.
1409670109 2014-09-02 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by company man Joe Herbert that the Patterson 261 rig was currently drilling a 8.25 inch diameter hole at a depth of 11,419 feet in the lateral. After walking around the site I noticed that the chemical containment liner had a large hole (6 inch diameter) in one corner. There were noticeable amounts of chemicals within the containment including oils and powders outside of their own containers. The containment was located over the French drain system which is believed to be in the shut position. I notified Joe Herbert of the containment issues and he made requests from contractors to immediately fix the containment and check the drain system to determine if it needed to be sucked down. Joe plans to sucks up all liquids within the containment system and fix it.
1965695177 2014-08-25 Drill / Deepen / Reopen No Violations On the date of inspection I spoke with the company man on site, Rodney Bean. I was informed that the Patterson 261 was drilling its 8.25 inch diameter hole for its production string. It was at a depth of 5857 feet at the time of my inspection.
1966128143 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.
