0239902117 2014-11-17 Administrative Inspection No Violations I received phone calls from Chespeake consultants Gene Belesom and Chris Chandlee. Gene informed me that his contractors would be spudding the conductor hole sometime today, 11-17-14. He plans to cement the conductor the following morning. Chris Chandlee informed me that he would be one of the consultants on the Nomac 337 rig that would be rigging up after the conductor is set. Chris said they would be moving equipment on site this week and could be spudding in as soon as 11-22-14.
0253633902 2014-12-15 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant, Chris Chamblee, that the Nomac 337 rig was drilling its lateral at a depth of 11,955 feet with a Kick Off Point of 8307 feet. For this well a pilot hole was drilled to a Total Vertical Depth of 9400 feet. A plug was set from 9400 to 8307 feet on 12-9-14. The details of the plug job will be included in my plug back report. I was also on site to ensure that the two Oil Based Mud (OBM) spills that occurred mostly in their secondary containment had been entirely cleaned up. The 3bbl OBM spill at the back frac tank and the 10bbl OBM spill under their shakers were cleaned up.

0644721909 2013-03-04 Not Drilled No Violations This location has a permitted well on it. There is a road leading to a large open field were the well was permitted for. It appears at this time that there is no action being taking in preparing the site for the drilling of the well. The permit is set to expire in November 2013 and a pad and the location have not be prepared for drilling, nor has a presite beeen requested at this time.
0769582453 2015-04-23 Completion Testing No Violations On the date of inspection I found that the stimulation of the well has been completed. I was informed by Jody Jones with Chesapeake that stimulation has been completed on 4-20-15. At the time of inspection there were still water tanks on location. I am unable to pass the site for Preliminary Restoration until vegetation has been established.

1156967725 2018-01-16 Preliminary Restoration No Violations On the day of my inspection I found pad containing one well. Production equipment includes a sand seperator, gas production unit, a 400 barrel steel tank, and pipeline equipment. Production records are current through 3rd quarter 2017. Site meets requirements of ORC 1509.072A for preliminary restoration. Locks on both gates were frozen and I was unable to view pressures of wellhead gauges.
1230631936 2014-11-18 Casing String Witnessed No Violations On the date of inspection I was informed by Chesapeake consultant Gene Belesom that Top Hole Drilling had drilled a 36 inch diameter hole to a depth of 120 feet. Top Hole Drilling set 30 inch diameter, 147lbs/ft conductor pipe on bottom. Express Energy Services pumped 330sk of Class A cement, 15.6ppg, 1.18 yield, 2% CaCl, and 0.50lbs/sk of Flo Seal. Express displaced cement with 91.4bbl of fresh water. 25bbl of cement returned to surface and stayed at surface while I was on site.
1416850878 2014-11-24 Casing String Witnessed No Violations On the date of inspection I am on site to witness the mine string. I was informed by Chesapeake consultant Chris Chamblee that the Nomac 337 rig had drilled a 24 inch diameter hole to a depth of 209 feet. Chris reported a soft section from 147 feet to 150 feet losing partial returns. The casing crew installed 20 inch diameter 94lbs/ft casing to a depth of 197 feet. Baskets were installed at 141 feet and 146 feet. Due to only losing partial returns, Chris will have Schlumberger attempt to establish circulation so that the section can be cemented conventionally. Schlumberger was unable to establish circulation by pumping fresh water to the capacity of the hole plus 10bbl. Schlumberger will now perform a balance job. Schlumberger pumped 242sk of Class A Cement to the bottom of the casing and mine void and pumped 10bbl of fresh water behind the cement. Schlumberger topped the annular space off with 320sk of Class A cement with returns at surface.
1417619317 2014-12-03 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Chesapeake consultant, Richard Clodfelter, that the Nomac 337 rig had drilled an 8.25 inch diameter hole to a depth of 7200 feet. At the time of my inspection the rig was in the process of tripping into the hole. I reinspected the area where there had been an Oil Based Mud (OBM) spill the previous week. The frac tanks had been cleaned up, at least one of the holes in the containment had been corrected, and within the containment had been cleaned of OBM. There is still one small location (approximately 1 foot by 1 foot) that there was OBM present outside of containment. I informed Chesapeake Drilling Superintendent, Cody Johnson, that until the OBM was entirely cleaned up outside of the containment that I could not clear my Spill Report. Cody informed me that he has dispatched his liner repair group to the site to lift up the berm of the OBM containment liner and ensure that there are no more holes in the liner. Cody also instructed rig hands to shovel up the OBM on the gravel. I will revisit the site after the final cleanup and repairs to ensure that there are no more visible leaks or OBM on surface.
1417894361 2014-12-05 Drill / Deepen / Reopen No Violations On the date of inspection I was on site to ensure that the 3bbl Oil Based Mud (OBM) spill that had partially left containment had been cleaned up. At the time of my inspection there was still a small amount of OBM outside of containment. It appears that the containment is still leaking onto the well pad below. During my inspection I was made aware of a second OBM spill that occurred at the half round below the shakers. The spill was reported as 10bbl and was reportedly caused by the proper valves not being closed by the BOS Solutions worker. I was not made aware of the spill until I had arrived on site to inspect the previous spill. I spoke with Chesapeake drilling consultant Richard Clodfelter and Chesapeake safety representative Ken Thorne. I instructed Ken Thorne to notify me of any spills in the future whether they believe they are on containment or not. The 10bbl OBM spill was reportedly on containment and Global Oilfield services was on site with a super sucker to remove the OBM from the containment. I accompanied Richard Clodfelter to point out the remainder of the previous 3bbl spill and he agreed to lift up the liner again and clean out the remainder of the OBM below as well as repair any leaks in the containment they can locate. The previous 3bbl spill is approximately 90% complete. After Global has completed the cleanup of the new 10bbl spill I will revisit the site to ensure that any OBM inside and outside of containment is cleaned up.
1456413296 2016-02-25 Production Wells No Violations My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #6H well has been drilled but not in production. No pressure gauges were found on the wellhead tree. Concrete barriers were in place around the wellhead. Well identification number was not found on the well but was on signage at entrance to access road. No production equipment nor tanks have been placed on the pad. The only other equipment found on the well pad was one roll-off waste container.
1474981761 2016-09-27 Plug / Plug Back No Violations On the date of my inspection I found a well maintained lease road with a fence and gate. The pad has 1 unidentified drilled well. The pad is surrounded by healthy vegetation. The wellhead is equipped with a christmas tree valve. There are no pressure gauges tied into the wellhead. Production records indicate the well has never produced and is a stratigraphic well. There is also currently a roll-off trash container on the pad.
