0043208545 2012-12-31 Drill / Deepen / Reopen No Violations I stopped at this well to discuss the logistics of NOMAC 38 moving to the Booth 23-11-4, well number 6H pad location. The permit for this well is on rush status and should be issued today. The location should be in mine area and I wanted to try to confirm that the rig is aware of this issue. We also discussed the drilling of the conductor and the need to have heavy snow removed from the pad. They are drilling ahead currently at 10,000. There are no violations at this time.
0075334966 2016-08-19 Production Wells No Violations On the date of my inspection I found the access road gate locked and the access road in good condition. Concrete barriers were in place around the wellhead. Proper identification was posted. The equipment on the pad consists of two 400 barrel storage tanks contained in steel dike, an upright sand separator, and a gas production unit. The well is equipped with a christmas tree valve going from 20 inch conductor to 13.375 inch casing to 8.625 inch casing to 5.5 inch casing. Annular pressure on the 13.375 inch casing was 0 pounds per square inch (psi), annular pressure on the 9.625 inch casing was 200 psi, pressure on the production 5.5 inch production line was 209.1 psi. Production records are current.

The well has been recently hydraulically fractured, the frac wellhead is still installed and containment is set around the location. Frac tanks are still on location following the job. New erosion controls have been installed as well. There were no workers or contractors on location at the time of the inspection. There are no violations at this time.

Express Energy is currently drilling with an auger rig and should complete the hole later today and cement the conductor pipe tomorrow. They were at 96 feet with a 26 inch hole and 74 feet with the 40 inch hole. This well will include a mine string. The location has no violations at this time.
1355450592 2012-12-27 Drill / Deepen / Reopen No Violations I stopped by the location to check the status of NOMAC 38 drilling rig. They have currently started their kickoff at 7859 and plan to TD the well at roughly 14500. They are also currently waiting on the next location that the rig will be moving to. They are no violations at this time.
1355878802 2012-12-14 Drill / Deepen / Reopen No Violations Nomac 38 informed me that they were going to be rigging up to begin drilling on the McCoy pad. At the current time they were rigging up while having some issues with the top drive. They had removed all equipment and supplies from the Wagner 5H pad and had an estimated spud date of Saturday 12/15. During my visit we also went over the procedures for running a mine string in the even that one was encountered as was state in the well permit. The pad had a 1-2 layer of soft slag on it from the rainfall but had no containment issues or violations at this time.
1356369979 2012-12-23 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 9 5/8 intermediate casing set at 1950 feet. 15.6 lb/gallon cement was circulated to surface with 14.7 lb/gallon cement returned. The floats held and there no violations during this time.
1359401396 2013-01-24 Administrative Inspection No Violations I conducted an inspection of this well site for complaint #4204 regarding the smell of propane/methane at a residence 2000 feet from the well. The well has been drilled but no fractured yet. The well is currently capped over. I took gas readings with my gas meter and received no readings of any kind. The well is shut in and not leaking. The location is slagged and there are no violations at this time.
1365014293 2013-04-03 Production Wells No Violations This inspection was a follow up to complaint #4204 regarding the smell of gas at a nearby residence. The well location is unchanged from the last inspection. A shut in, capped well is on location surrounded by fencing waiting to be hydraulically fractured. Also a crew was onsite to install monitoring cameras for surveillance in Oklahoma City. There were no signs of gas and my Sperian gas meatier read 0 for any methane. I followed up with Mr. Jakakai and was informed that the smell of gas was coming from an old hot water heater not any nearby oil and gas wells. This complaint can now be closed.

Pad is stable and vegetated.
1890382072 2016-01-26 Production Wells No Violations On my inspection today I found one well in production with Christmas tree valve assembly, with tubing and annulus gauges. Production equipment for the site includes one separator and two 400 barrel production tanks that are located within properly sized steel containment. The earthen berm surrounding the pad is in place and vegetation has been established on all slopes. The location is fenced and has proper I.D. signage on the tank, wellhead and access road entrance.

Currently the rig and all Nomac trailers are offsite. A few pieces of equipment and Great Plans mud tanks are currently left on site. The well is capped and awaiting completions. The pad is muddy and wet but there are no violations at this time.
