The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. A stable earthen dike that was free of fluid was present around the production equipment. A locked chain-link fence enclosure was present on top of the dike. Privacy screening on the fence was deteriorated. Correct identification was present on the fence. Equipment on location consisted of one (1) vertical separators and one (1) 140 bbl production tanks. The separators remained in good repair. No production charts were present. The production tanks remained in good repair. Valves were closed and plugged. A charcoal filter was present. Hatch lids were closed. No violations were observed during this inspection. |

The well was located within a screened and locked chain-link enclosure that contained correct identification. The screening was worn. Evergreen trees were present to shield the location from view. The 8.625 well head was at grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. Staining consistent with a legacy operation was present. A stable earthen dike that was free of fluid was present around the production equipment. A locked chain-link fence enclosure was present on top of the dike. Privacy screening on the fence was deteriorated. Correct identification was present on the fence. Equipment on location consisted of two (2) vertical separators and two (2) 100 bbl production tanks. The separators remained in good repair. A current 16 day production chart was present. The production tanks remained in good repair. Valves were closed and plugged. The load out valve on the South tank did not contain a plug. At the time of my inspection I witnessed production brine dripping from the valve. No product was collecting on the ground or was mobile. A charcoal filter was present on the South tank. Hatch lids were closed. While on location I called and spoke with Duck Creek Energy representative Larry Gibler regarding the production tank leak. I communicated to Mr. Gibler that I was issuing a compliance notice due to well operations causing contamination and pollution. I requested that the plug be installed within five (5) days. I will schedule a follow-up inspection to document the status of repairs. |
