0117125827 2013-06-11 Drill / Deepen / Reopen No Violations Due to the size of the location and that this is an urban urban area. I instructed Ted Hill from Ohio Valley Energy that we needed to test dig an area where the pits are gone to be constructed to confirm the integrity of the soil. We dug an 8 hole. The integrity was good, clay no rocks on water.
0156454704 2014-08-06 Production Wells No Violations I was on site to conduct status check with Inspector John Fleming. The site is urban. The wellhead and tank battery are fenced in and locked. A dike surrounds the tank battery which consists of a 140 bbl production tank equipped with a spark arrestor and charcoal filter, a 28 bbl brine tank, and a horizontal separator. The wellhead is equipped with an electric pump jack.

I meet Ted Hill from Ohio Valley Energy and we walked the location before they stated cutting and clearing the trees. All stumps and branches will be chipped and hauled from location, logs will be stacked for land owners use. ID signs are posted at the entrance on 12th Street.

Universal Well Service established circulation and pumped 250 sacks of 10-4-2 Unitropic cement and displaced with 87 barrels of fresh water plug landed with 1650 psi. Pressure was released after 5 minutes plug held. There was good circulation though out the job.

Plug down at 1720 with 14 barrels of cement returns.

At this time the well is not in production waiting on Columbia Gas. The tank battery is equipped with one 100 barrel tank and one 35 barrel water tank and one horizontal separator. Vegetation has been established. The wellhead is equipped with a pumping and an electric motor. ID signs are posted. No violations were noted at this time. The Preliminary Restorations is passed.

Petroset was on location to cement the casing, they established circulation. Pumped 340 sacks of class A cement with 3% CaCl and 1/4# flake, displaced with 20 barrels of fresh water. Plug down at 10:47am. There was 20 barrels of cement returns.
1375284191 2013-06-21 Drill / Deepen / Reopen No Violations I performed a status check on the above location. After setting the intermediate Wild Cat Drilling switched from drilling on fresh water mud to air. There was a waver approved for this. All dust was controlled with a pit liner layer over the pits. There is 400 barrels of salt brine kill water on location. Permits are posted in dog house. No violations at this time.
1377182010 2012-07-23 Urban Drill / Deepen / Reopen No Violations I was location to witness the fracing of the above well. Producers Well Service was on location to perform the fracing of the above well. All safety meetings were conducted, danger areas were marked with yellow ribbon, fire extinguisher were in place. Annuls was open. During the operation there was no pressure on the annuls. This operation was completed without any incident.
