0046482989 2014-04-03 Drill / Deepen / Reopen No Violations Visited location on 04/03/2014 to witness logging job on the Bleachtech #10 well. The hole was loaded on 04/02/2014 with pit water (note: drilling operations had been completed the previous week, so solids in pit had settled out and water was taken from top of pit). Appalachian Well Services rigged up and logged the well. The logging tool was run without centralizers, as they were damaged on the previous run. This caused the collar locating tool to not work properly, but the VDL tool was not effected. The cement bond on the 8 5/8 casing looked excellent throughout the entire length of the string.
0198112094 2014-03-12 Drill / Deepen / Reopen No Violations Visited location on 03/12/2014 to witness the production casing cement job for the Bleachtech #10 well. An 11 hole was drilled to 2,440 and 8 5/8 casing was run to 2,406. The well was cemented with 650 sxs of Class A cement with 2% gel and 6# gibsonite. Good circulation was maintained throughout the job, and 8 bbls of good cement returns were seen at surface. Witnessed MIT test on production casing immediately following the cement job. Well was pressured up to 500# over landing pressure and held for 15 minutes; no noticeable pressure was lost throughout the duration of the test, so the well passed. Plan forward is to wait on cement, and then drill out to TD.
0413070693 2014-03-08 Drill / Deepen / Reopen No Violations Visited rig on 03/08/2014 for surface casing cement job. Poulson Drilling drilled 14 3/4 hole to 661, and ran 631 of 11 3/4 surface casing. Rigged up Petroset to cement casing to surface. Cement job went well; 7 bbls good cement returns to surface.
0417021670 2017-05-18 Solution Mining Project No Violations Inspected well on 05/18/2017 with Inspector Steve Ochs at the request of Andrew Adgate to confirm the well status. The well has been placed into production, however no well identification is posted. The operator will be contacted and advised to post it promptly. The well status (AI) is correct. No other problems were noted at the time of the inspection.
0456738031 2014-04-02 Drill / Deepen / Reopen No Violations Visited location on 04/02/2014 to top off wells with a mixture of mortar, quick-crete, and Portland cement. Cement had fallen approximately 4 - 8 on all casing strings. Mixture was hand-mixed on location and dumped down the annulus and filled up to surface.
