The pipe was cemented back to surface with 100% excess ran and the tin horn cellar sealed 12 deep at the same time. Express Energy Services were the top hole drilling contractors.The well was spudded on 03/01/13.

0062351581 2019-01-24 Production Wells No Violations On this day, I am conducting an inspection to document the SIMOPS progress for the plug and abandonment of permit number 4446. I spoke with Antero representative Barney Etcher, and he stated the SIMOPS process is being delayed due creating a standard operating procedure between departments within Antero Resources. Containment is being set in place and steel cages for the producing wells are onsite. The well in question currently has a nightcap in place and no gauges are present. I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units (GPU), (4) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.

Addressing the well status the well drilling and completion process is complete with the well in production with additional gas processing equipment located on the well pad in a separate location from the producing wells, there is also a total of 8-400 barrel capacity production fluid tanks on location located inside a plastic lined metal dike area with proper well identification posted at the well head.

0482809979 2014-03-13 Production Wells No Violations Inspector Eric Conrad and myself met on site with Anteros production supervisor Daren Rader discussing several site concerns listed as follows, site entry point identification, proper site soil compaction over the recently installed production flow lines, procedures for identifying temporary storage tanks with proper liquid content identification, general procedure safety during the completion drill out process, proper mapping and identification of buried underground production lines, proper containment under all fluid vessels on site. Mr. Rader understood this was to be addressed as procedure concerns on all Antero sites in general.
0517150552 2018-03-07 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 150 pounds zero pressure and the 9.625 casing well head reads 700 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area.

0742388077 2013-10-29 Production Wells No Violations Follow up review to complaint # 4517. On this day Inspector Eric Conrad and myself did a calculated approximation of the gas production and found the 2 wells involved in the complaint to be meeting the minimum standards of gas production. Our findings were as follows, The chart is a 30 day 100# chart on a 500# meter with a approximate line pressure of 3 psi =.75 mcf or 750 cubic feet of gas produced per day x 365 days in a calendar year = 273 mcf of produced gas per year meeting the State minimum of 100 mcf of gas per year per well.
0953108746 2013-04-14 Administrative Inspection No Violations Notification was made to the inspector that the initial conductor cement job was communicating to surface when they started to drill the surface bore hole. The conductor was re cemented using 704 sks of Class A 2% calc, 1/4# flake -15.6 # 1.18 yield cement with returns to surface.

1114679041 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the front right side of the pad, and a midstream facility along the front right side, as well.
1362233077 2013-02-27 Drill / Deepen / Reopen No Violations Inspector called out to set 100 of 26 conductor, Upon inspection I discovered the surveyors had set the proposed well stake only 6.5 from the existing well on the pad with the approved permit plans showing a 15 spacing between the wells on the pad. I advised them they could not used that surveyed mark and they would have to get the surveyors back out and re stake the well.
1366123367 2013-04-16 Drill / Deepen / Reopen No Violations Inspector on site 10 hours to witness the running and setting of 340 of 18.625 surface casing. The casing was set and cemented back to surface using Class A- 2% calcium and 1/4# flake with 100% cement ran and 85 barrels of cement returns to surface, with the casing being pressure tested to 500 Psi with no recorded pressure loss.
1366753362 2013-04-23 Drill / Deepen / Reopen No Violations Status check inspection of the current drilling progress found the current drilling depth to be 3201 and a wire line company currently doing a borehole deviation survey. The next casing string to be set will be the 9.625 intermediate casing string at 6950. I also received the 13.375 intermediate casing string paperwork records from 04/20/2013.
1367242696 2013-04-28 Administrative Inspection No Violations I received phone notification and was in phone contact with the company man on site during the running and setting of the 9.625 casing string and casing cement job. The current well bore stage drilling depth was reached at 6,951 using a 12.250 drill bit and 6,912 of 9.625 casing was ran and cemented with a cement top @ 500.
1367600435 2013-05-03 Drill / Deepen / Reopen No Violations Status check inspection found the current drilling depth to be 12,599 in the horizontal borehole section using a 8.5 drill bit and using oil based drilling fluid. The projected well bore total depth is 15,078.

The well is has been completed and the testing and completion process is finished and the well is hooked up awaiting the completion of the on site processing facility and gas transportation lines in area to be complete.
1380726541 2013-10-02 Preliminary Restoration No Violations Status check inspection on the well and location, On this day I spoke with Anteros safety coordinator and asked that each well head have proper identification and proper identification be placed at the entry point of the location. There are also contractors on site emptying the earthen pit and preparing to close it in. The area around the site has vegetation established and the drilling equipment has been moved off with the well currently in production. The tank battery has been erected on the site pad. 8 - 400 barrel storage tanks are on site.
1382911916 2013-10-27 Production Wells No Violations Follow up inspection to complaint # 4510 forwarded over from Aaron Wolfe with the Ohio EPA, The photographs sent to the EPA by Ruth Partin were reviewed by my Supervisor Rocky King and Myself and along with the EPA it was found no violations were present in the photos, The request by Ruth Partin to disclose the nature of the production equipment on location will be for site safety reasons forwarded to Anteros public relations department for answering, The EPA provided only a email address and failed to document a time or day of contact as well as any personal contact information for Ruth Partin. My on site inspection found my recent request after my last inspection on 10/02/2013 to post proper identification met, All 3 wells on location are in production and a Myers tank truck was on location withdrawing processed water with proper safety grounding and spill containment in place while loading, The Myers UIC number was also properly posted on the tank truck.
1384561900 2013-11-15 Production Wells No Violations While on site at the Carpenter drill pad I was notified by the safety man that they had a small fire on the Rubel production pad, I was on site within 6 minuets and no fire was observed and it was discovered that a hammer union on a excess gas burner had become loose causing a small amount of gas to escape and flash when the burner ignited, The hammer union was tightened and the company agreed to install safety devices between the production tanks and the safety burners as not to allow any liquids to migrate from the tanks to the burners of allow any potential hazard to back feed into the production tanks through the gas escape ventilation system.
1728252504 2013-06-04 Completion Testing No Violations Status check inspection on the completion process found the company has finished stimulating stage 16 and has begun stage 17 with a completion bridge plug set at 10,940 and the well casing perferated with 60 shots from 10701 to 10,890.
2129218668 2019-01-29 Production Wells No Violations On this day, I found Antero progressing with the SimOps procedure in preparation for the plug and abandonment of permit number 4446. TekSolv is onsite installing back-pressure valves in the tubing hanger on the (6) producing wells on the pad, prior to installing the steel cages over the wellheads. Fortis Energy is also onsite installing containment and preparing to rig-up, once the SimOps procedures have been completed. I spoke with Antero Representative Barney Etcher during the inspection.
