0060339106 2014-01-27 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current drilling depth to be 6,042 in the 12.250 2nd intermediate well bore section with a proposed total depth to be drilled of 7,149. The company is misting 3% KCL.
0107011558 2014-02-03 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the current drilling depth to be 7,996 and the company beginning the horizontal well bore curve section with a current azimuth of 38.18 degrees and inclination of 65.48 degrees. The company is operating on a 21 day blow out preventer test cycle time period.
0427339870 2014-01-22 Drill / Deepen / Reopen No Violations Status check inspection found the current drilling depth to be 3,349 in the 12.2502nd intermediate well bore section with the company using directional drilling tools with a inclination of 4.76 and azimuth of 86.26 degrees. The company has completed the required deviating for this section and is preparing to pull out of the hole with the directional tolls and pick up a 12.250 hammer bit and run back in the hole and complete this well bore section with the next casing string to be set being 9.625. On this day I also addressed several issues with the ground of rig support equipment.
0441854420 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the back of the pad, and a midstream facility behind the gas processing units.
0540574392 2014-05-01 Completion Testing No Violations Status check inspection found the stimulation process complete with 32 stages stimulated by Go-Frac well services and the company rigging down and moving over to begin the stimulation on the Gary # 1-H well API # 3411124375-0000.
0817697556 2014-07-30 Preliminary Restoration No Violations Status check inspection on the preliminary restoration found the well drilling and stimulation process is now complete and the well is in line and producing with 8- 400 barrel production tanks on site servicing all 6 wells on the drill pad now in production.
0834612231 2019-01-29 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
1249091200 2018-03-07 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 200 pounds pressure and the 9.625 casing well head reads 0 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
1383439976 2013-11-02 Drill / Deepen / Reopen No Violations The said well was spudded on 11/02.2013 with 100 of 26 conductor to be set inside a 32 well bore. Fresh water was hit at 4 while setting the cellar.
1384560714 2013-11-15 Drill / Deepen / Reopen No Violations Inspection found the current drilling depth to be 1164 using a 17.5 bit with the next casing point to be 2000, Gyro Deviation borehole surveys are being taken every 250 with the last survey at 1033 showing a inclination of .31 degrees.
1384799877 2013-11-18 Drill / Deepen / Reopen No Violations Status check inspection found the drilling company Falcon rig # 20 has completed the drilling and running of the 13.375 1st intermediate casing string with 2070 of 17.5 hole drilled and 2070 of 13.375 casing set inside of a 17.5 borehole with 73 barrels of cement returns to surface, inspector did not witness the surface cement job due to being in a mandatory training meeting on 11/17/2013.
