0093503606 | 2014-03-03 | Drill / Deepen / Reopen | No Violations | Status check inspection on the drilling progress found the current drilling depth to be 7,565 in the horizontal curve section using a 8.5 directional bit assembly. |
0341192176 | 2014-01-02 | Drill / Deepen / Reopen | No Violations | Status check inspection on the drilling process found the current drilling depth to be 2558 with the company currently drilling a 8 degree borehole deviation with a borehole deviation survey being taken every 90. |
0413534985 | 2014-01-06 | Drill / Deepen / Reopen | No Violations | Status check inspection found the current well bore 12.250 2nd intermediate drilling section to be complete with a total depth of 7,002 drilled. |
0542244706 | 2014-07-30 | Preliminary Restoration | No Violations | Status check inspection on the preliminary restoration found the well drilling and stimulation process is now complete and the well is in line and producing with 8- 400 barrel production tanks on site servicing all 6 wells on the drill pad now in production. |
0776076199 | 2019-01-29 | Production Wells | No Violations | On this day, I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 50 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 200 psi. |
1110277868 | 2014-03-10 | Drill / Deepen / Reopen | No Violations | My inspection today finds the rig is running drill pipe into the hole. The well is a drilled to a depth of 13396. |
1351977854 | 2013-11-30 | Casing String Witnessed | No Violations | On 11/30/13- 380 of 18.625 surface casing was ran by Falcon rig # 20 and cemented to surface by Allied cement services with 60 barrels of good cement returns to surface and the casing pressure tested to 500 psi over lift pressure. |
1371643091 | 2013-12-13 | Drill / Deepen / Reopen | No Violations | On site inspection to document the status of the drilling process, The well bore has been drilled to the point of the 1st intermediate casing string and the larger rig is slated to move in soon. |
1386115634 | 2013-12-03 | Drill / Deepen / Reopen | No Violations | Inspection found the current drilling depth to be 770 using a 17.5 hammer bit with the next casing point to be 2062 setting 13.375 casing. The drilling rig is currently broke down and being repaired. |
1417943706 | 2014-03-05 | Drill / Deepen / Reopen | No Violations | My inspection finds the company has installed a water diversion berm at the top of the slip. The rig is currently drilling at a depth of 9038. |
1489694981 | 2018-03-07 | Production Wells | No Violations | On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the back of the pad, and a midstream facility behind the gas processing units. |
1739562674 | 2013-12-12 | Drill / Deepen / Reopen | No Violations | Wells have been spudded but not completed. Waiting on drill rig for drilling intermittent strings and lateral. |
1751967974 | 2013-11-01 | Drill / Deepen / Reopen | No Violations | The said well was spudded on 10/31/2013 with 100 of 26 conductor to be set and cemented to surface. Fresh water was encountered at 4 from surface. |
2094058700 | 2018-03-07 | Production Wells | No Violations | Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 200 pounds pressure and the 9.625 casing well head reads 0 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair. |
