0297475961 2014-01-27 Casing String Witnessed No Violations Status check inspection on the drilling progress found the 12.250 2nd intermediate casing string total depth drilled to 7341 and Allied Cement services on location cementing the casing with a estimated top of cement to be 665.
0960330566 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units, (4) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. This well is setup to be produced by means of a plunger lift system. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. It will be noted that in 2018 a slip had occurred along the southern slope of the pad and western slope of the lease road, prior to the pad. The slopes have since been graded, seeded, and erosion control in place. Currently awaiting new vegetation.
1218889603 2014-01-15 Casing String Witnessed No Violations On 01/15/14 the 17.5 diameter 1st intermediate borehole total depth drilled was reached at 2268 and 2267 of 13.375 J-55 54.5# casing was ran.
1382237677 2018-03-07 Production Wells No Violations Status check inspection found the well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 0 pounds pressure and the 9.625 casing well head reads 1,700 pounds pressure. On pad total is (8) complete wells supported by (8) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
1394744340 2014-03-13 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the current drilling depth to be 11,219 in the horizontal 8.5 production well bore stage using a 13.3 pound and 85 viscosity oil based drilling mud achieving a drilling rate of penetration of 117 per hour with a current directional azimuth of 333.70 degrees and inclination of 92.37 degrees.
1395174180 2014-03-18 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson drilling at a current depth of 16,693 in the horizontal well bore section using a 8.5 diameter drill bit with a true vertical well bore depth of 8,301.68 and current directional azimuth of 331.43 degrees and inclination of 90.74 degrees.
1404813177 2014-07-07 Completion Testing No Violations Status check inspection of the completion process found US Well service on location to stimulate the well and Renegade Wireline service on location to perforate the well bore, a walkthrough inspection was conducted of the site with the company man on duty and the Antero Safety man on duty, It was pointed out that the Renegade wireline company had no signage indicating explosives were on site and the vehicles containing the explosives were not marked, this was addressed with the company representatives on site, we also discussed and reviewed the MSDS sheets and the need to have the companies keep an accurate inventory of the Hazardous Materials and their location on site. The site stimulation process had not yet commenced and a mandatory personnel on site drug test was being conducted by Antero on this day.
1405974197 2014-07-21 Completion Testing No Violations US well service on location stimulating stage 50 of 52 with 60 Perforations shot in 6- 10 shot clusters between 8,988 and 9,136 in the horizontal well bore section with a completion plug set at 9,150, the annular pressure is being monitored electronically.
1410891362 2014-09-16 Completion Testing No Violations Status check inspection of the well completion process found the well stimulation process complete and Key Energys work over rig on location rigging up to drill out the completion plugs. Chloride levels reading 700 parts per million were found on the well pad around the 500 barrel open flow back tanks, the company was asked to decrease their pressure when exhausting line pressure into the tanks. 8 -300 barrel production tanks have been installed in a established man made containment dike area.
1573405354 2014-01-10 Casing String Witnessed No Violations On 01/10/14 Patterson rig # 332 is now on the hole and 472 of 18.625 surface casing was set and cemented o surface by Allied cement services with 932 sks of class A cement with the casing being pressure tested to 800 psi over lift pressure upon the landing of the plug, cement returns to surface with 100% cement excess ran.
1718400127 2014-01-13 Production Wells No Violations Status check inspection found the current drilling depth to be 1660 with a proposed total depth for this 17.5 1st intermediate well bore section to be 2,268.
1948488733 2013-11-01 Drill / Deepen / Reopen No Violations The said well was spudded on 11/01/2013 with 100 of 26 conductor being set and cemented to surface. Fresh water was encountered at 68 from surface.
960330566 2019-2-7 0:00:0 PW No Violations 0.10
