0012224760 2015-05-11 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the following present on location- Stingray Pressure pumping services are on site along with Nine Energy wireline implementing the completion process on all 4 wells on location that have been drilled. All stimulation support equipment is on containment with the well pad berm serving as secondary containment with all site valves in the closed position during stimulation. The company does have personnel in place monitoring and testing any fluid that may accumulate before discharging takes place. Addressing the Preliminary Restoration the well pad construction phase is complete with adequate vegetation and erosion control being established in the affected areas, the well pad working surface shall remain as stone. 4-400 barrel production fluid storage tanks are present on location and all production flow lines are in place with some fine pluming connection to take place during the final completion phase. Preliminary restoration passed on this day.
0424000578 2014-06-17 Casing String Witnessed No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed Rocky Mountain Drilling Inc. drilled a 32 inch diameter hole to the depth of 110 feet and ran 110 feet of 26 inch 102.63#/ft. conductor pipe. Stingray Cementing was on site and gained circulation with 27bbls of fresh water. Stingray pumped 15bbls of gel, 10bbls of fresh water and cemented pipe with 273 sacks of class A cement with 2% CaCl and .25#/sack of Celluseal. Displaced cement with 65bbls of fresh water had good cement returns of 20bbls and cement stayed surface.
0962609151 2015-07-22 Completion Testing No Violations Status check inspection on the well completion process found the well is now capable of production and Pro Flow testing service is completing the final flow testing process. 4 Gas processing units are present on location to support the existing wells.
1042278534 2017-02-13 Production Wells No Violations Status check inspection found the well in production and selling off the 2.875 production tubing tree. The well head did have annular monitoring gauges in place with zero pressure on the 13.375 and 200 psi on the 9.625 annular. The well is producing into a gas processing unit and selling into a main sales line. There is a tank farm consisting of a total of 4 tanks with the capacity of 400 barrels each with the tanks located inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads and location entry point. The company has also installed emergency shut down devices at each well head along with at each gas processing unit and at the location entry point and production tank farm.

1408715998 2014-08-22 Casing String Witnessed No Violations On site inspection for a 8:00am callout to witness the running and cementing of the 13.375 surface casing with Sidewinder rig 209 on the hole drilling the surface borehole to a depth of 350 and running 348 of 54.5# 13.375 casing, CalFrac well service cement crew is on location to cement the casing using 514 sacks of class A 2% calcium .25 pound per sack cement with 50% open hole excess ran. At 2:45pm I left location and shall have another inspector cover the job now scheduled for 7:00 pm due to a default in the cement head.
1409096055 2014-08-22 Casing String Witnessed No Violations On site to witness Sidewinder Rig 209 run 11 joints of 13.375 surface pipe and Calfrac cemented it in place with 515 sacks of Class A cement and they had 46 bbl of good cement returns and it stayed up good.

Note: Verified cement mix water to be fresh water and they had cement shoe on bottom and float collar on top of the first joint of casing in hole and they ran four centralizers.

1448242673 2015-06-23 Completion Testing No Violations Status check inspection of the well completion process found Mercer Well Service rig on location and has recently completed the post drilling well bore clean out run with 2.875 production tubing set to a depth of 9,929. Pro FlowCompany flow back was on site to provide well flow back operations and testing. The production well head equipment is now in place and the crews are mopping up and preparing to move over to the McLaughlin well pad.
1514414289 2014-06-17 Casing String Witnessed No Violations On the date of inspection I accompanied ODNR Inspector Doug Bennett to witness a conductor cementing job. Refer to Doug Bennetts report for further details.
1583438581 2014-08-27 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the 1st intermediate well bore has been drilled to a depth of 2,385 and 36 # 9.625 casing ran with CalFrac cement services cementing the casing using 799 sacks of class A cement with 23 barrels of cement returns to surface reported, the blow out preventer was installed on the 13.375 casing string and the company is beginning to drill out from under the 9.625 casing.
2017858331 2019-04-10 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells equipped with a plunger lift system, (1) gas processing unit, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 200 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current. I spoke with Company Representative Steve Habig during the inspection.
2023410844 2014-09-04 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found Sidewinder Drilling rig #209 on the hole drilling the 8.5 diameter production borehole at a depth of 14,107 in the horizontal well bore section with a proposed total well bore depth of 16,840 and true vertical depth of 9,695 along with a directional azimuth of 155.58 degrees using a 11.5# drilling mud weight.
2103540027 2014-09-08 Casing String Witnessed No Violations Status check inspection on the drilling process found Sidewinder rig 209 on location with the 8.5 well bore total drilling depth recently reached at a depth of 16,841 with a true vertical depth of 9,757 and direction azimuth of 140.77 degrees, the 5.5 production casing has been ran and set to a depth of 16,831 and Calfrac cement services is on location cementing the casing using a 2 part lead and tail cement mix.
