0240140076 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The intermediate casing has a pressure of 350 pounds per square inch (psi). No gauge is available for the surface casing. On 5/28/2019, I spoke with Company Representative Steve Habig to inquire about the presence of a surface string gauge. It will be noted that slight erosion has begun at a stockpile on the east side of the lease road, just prior to the pad. I spoke with ODNR Engineering Technician Andy Thomas to notify him.
0648098904 2015-08-11 Preliminary Restoration No Violations Prelimminary Restoration status check inspection found the area affected deplicted in the pre site construction has erosion control and vegetation in place. There are a total of (4) wells on this pad all named the Howell. Addressing the status of the wells all 4 wells are capable of production with a total of 4 gas processing units in place on locatin with one each supporting each well. Near the entry point of the location located on the well pad the production fluid storage facility is located with a total of 4 -400 barrel production tanks located in a poly lined steel containment dike area. Preliminary Restoration meets 1509.072 and is passed on this day.
0677167076 2014-12-16 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig 538 on location drilling the 8.750 vertical section on air and misting. No 2nd intermediate casing is to be ran and the company will switch to oil based drilling mud and a 8.5 directional assembly before starting the horizontal curve well bore section.
0946307380 2014-10-22 Casing String Witnessed No Violations The said well was spudded on 10/21/14 by Express Energys top hole rig with a 105 x 32 conductor borehole being drilled. On 10/22/14 Stingray cement service cemented the 26 x 108 of conductor pipe using 273 sacks of Class A 2% calcium .25 pps flake cement with the cement weight verifyied and mix water tested for chlorides and Ph level, good cement returns to surface.
1418195835 2014-12-10 Casing String Witnessed No Violations 12/10/2014-On site inspection to witness the surface casing cement job found the following. Patterson rig 538 is now on location and has drilled a 17.5 x 334 surface borehole and ran 332 of 13.375 casing with Calfrac cement service on location to cement the casing using 427 sacks of class A 2% calcium .25 pps flake 30% open hole excess cement with circulation established prior to cementing with a 40 barrel 6% gel sweep ran ahead and 8 barrels of cement returns to surface. The casing was pressure tested to 500 psi over lift pressure upon the landing of the plug.
