0015536735 2010-02-18 Administrative Inspection No Violations A complaint was received by the DMRM on 2/18/2010, alleging water well has caused animals to have tumors and daughter now experiencing seizures at 7095 Buttermilk Hill Rd, McConnelsville, OH. On 2/18/2010, a Ground Water Contamination Questionnaire was provided to the complainant, Ms. Annette Combs. The complainant states they boil water and noticed film around the pan resembling oil and water mixture; local and state Health Dept advised not to drink. Mr. Tom Tomastik, Geologist 4, and Mr. Don Grubbs, Morgan Co. Health Dept, 740-962-4572 were contacted on 2/18/10. Mr. Grubbs states: Touchy subject and beyond what he is supposed to sample. Health Dept Board Member, Dr. Downing rents the house, states water is good, and has even hauled water for tenant. Mr. Grubbs learned of concern before Thanksgiving 2009, no sampling/analysis, or investigation.
0074952562 2010-02-23 Non Well No Violations A water well complaint was received by DMRM on 2/18/10. On 2/23/10, an investigation was conducted at 7095 Buttermilk Hill Rd. During the investigation, it was noted that residence draws water from a spring fed well. A 4 yellow corrugated plastic pipe drains water out of hillside into a concrete holding tank. The tank lid did not appear water tight, due to crack and overall fit. Gas Detector did not reveal elevated levels. A possible fuel tank found 3/4 buried near holding tank. The pump was found covered with extension cord routed along tree and fence line. 3 PVC above ground was routed from top of hill to tank for hauling purposes. Confirmed no water being hauled at this time. The septic system was found behind residence and above water source at higher elevation. The approximate distance was 500. A driveway separates property on top of hill. Permit No. 1932 exists on opposite side of hill from water source. The valves for this well were closed, and gas lines observed. Two existing water wells exist on neighboring properties. Obtained samples from holding tank. The complainant displayed alleged oil and water mixture. Confirmed allegation is not oil sheen. Update: The lab results revealed this complaint is not oil field related.
