Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1426626649 2015-02-16 Final Restoration No Violations I inspected this abandoned well on 3/16/2015. Columbus request an inspection to determine if the plugged and abandoned well meets the criteria for the Final Restoraton designation. The previous well was located on the property at 499 South Main Street, Monroe Falls, Ohio. My inspection revealed that all the criteria for Final Restoration have previously been met. The well was P & Ad in 2008. The area is stable, free of erosional features and all equipment has been removed.
1556101387 2007-12-04 Production Wells No Violations The Uniontown office received a call from a consultant performing a Phase I site assessment at 499 South Main Street in Monroe Falls, Ohio. The well is located southwest of the house and is overgrown and has apparently not been used for some time, but did not appear to be leaking. It is possible that an orphan well application will be submitted for this well.
