0390580783 2016-05-09 UrbanProduction Wells No Violations As a Compliance Notice ( # 2120472420 ) follow up I re-inspected the Carter # 2 well and tank battery. The previous Compliance Notice detailed non-compliance issues with the fencing on the wellhead and tank battery. My inspection found that this issue was brought into compliance. Another non-compliance issue was the leaks on the separator and the ground contamination. While the leaks on the separator were fixed the ground contamination still remained an issue. I spoke with John Raberat ( Diversified Resources, 440.840.0455 ) today and informed that the Compliance Notice was still in effect and would remain so until the ground contamination was remediated. He replied that there was a miscommunication with his well tender, but it would be remedied within seven ( 7 ) days. I will re-inspect the well for complete compliance. I requested that a key to the lock be supplied or a change of locks to a more common key. John replied he would change the locks to a common key.
1021177752 2016-05-17 UrbanProduction Wells No Violations I re-inspected the Clark #1 & #2 tank battery to assess compliance with the previously issued Compliance Notice # 2120472420. The contaminated soil and gravel under the separator had been removed and replaced. The non-compliance issues had been corrected.
2120472420 2016-03-21 Production Wells Violations Noted The Carter # 2 well was completed on 10/1977. The well was constructed utilizing 8.625 surface casing ( 412 ), 4.5 production casing ( 3854 ) and 2.375 tubing. The well was operated as a plunger lift well. A review of the RBDMS data base indicated that the Operator was current in the reporting of the production volumes. The location of the well and tank battery was positioned within the Cuyahoga Valley National Park boundary. The fencing surrounding the wellhead was broken down and in need of repair. I did not identify any leaks or contamination at the wellhead. The tank battery consisted of one (1) 100 barrel oil tank, a horizontal mechanical separator and positive shut off drip. The I. D. signage was in place and legible. The load lines were secured with flat plugs and were tight. The tank battery was surrounded by a earthen dike and partial fence. It appeared that the fence was taken apart for removal of a tank and not reconnected. The separator was leaking crude oil from a two (2) inch open pipe on the west side of the separator. It was causing ground contamination. To bring the Carter # 2 into compliance the Owner / Operator shall fix the leaking pipe on the separator and remove the ground contamination ( 1501:9-1-07 Prevention of Contamination and Pollution ). The General Safety violation ( 1501:9-9-02 Safety Regulations ) shall be brought into compliance by the repair of the fence structures surrounding both the wellhead and tank battery.
