0236189783 2004-02-09 Production Wells Violations Noted During a random status check inspection, I discovered that both 140 bbl stock tanks had been overfilled and that crude oil and/or brine had been released into the dike area. The spill is estimated at less than five barrels. The tank battery is apparently used for three wells (P/N 0704, 0737 and 0745). I contacted Jay Henthorne at Petro who in turn directed me to Steve Franks (330-465-8866). Steve stated that brine had recently been hauled and that there was tank capacity. The spill will be remediated as soon as possible.
0277973977 2004-06-29 Production Wells Violations Noted The leaking 100 bbl tank has been taken out of service which has stopped recurring free oil inside of the containment dike. I told Steve Franks that the site needed to be finished within two weeks or I would request a CO.

The cause may be a rising water table which has pushed oil released in February, back to the surface. I contacted Steve Franks and requested that he meet me on site on April 9 to discuss remediation. The impact has gone beyond the tank battery and has affected a drainage ditch and grass 200 feet east of the tank battery. I contacted both Jay Henthorne and Steve Franks and ordered that the tank battery be removed and remediated and the impacted ditch and grass area be removed and restored.

1144864899 2006-04-07 Production Wells Violations Noted Status check inspection found that one of the stock tanks had been overfilled again resulting in oil spilled inside of the containment dike. The stock tanks need to be emptied, cleaned and removed prior to removing and properly disposing of the impacted soil and gravel in the containment area. This is the second time that this type of spill has occurred. The same thing happened in February 2004.

1952783291 2004-03-02 Production Wells Violations Noted On Monday 3/01, I met with representatives of Cuyahoga Falls, Petro Evaluation and Poff Excavating and explained what needs to be done to remediate the spill. The first action will be to empty and remove the stock tanks to expose the impacted soil inside and adjacent to the tank battery. The impacted soil will be removed and disposed. The tank battery will then be rebuilt and stoned. The surface affectment in the drainage ditch and the nearby lawn area will raked and removed prior to seeding and mulching. Work began today with the removal of the tanks and partial removal of impacted soil.
