1360440798 2015-01-15 Plug / Plug Back No Violations I was on the Morgan Adhesive # 2 location to observe the plugging and abandonment of the well. Moore Well Services removed the 2.375 tubing from the well. A RTTS R - 4 packer was found in the tubing string. The packer was used to eliminate the pressure differential between the Clinton sand perfs and the Newburg Dolomite perfs. The tubing was examined and deemed ok ( minor sulfide pitting and corrosion ) to run back into the 4.5 casing in order to spot the bottom plug. The tubing was of varied lengths 31 to 23 and the decision was made to tagged the bottom with the jars. The depth was found to be 3980 GL. No violations were identified.
1597747076 2006-12-26 Production Wells No Violations Status check inspection. This Morgan Adhesives well was drilled in 1980 and operated by KST Oil & Gas. However, KST is no longer is in business and there is no other owner identified. Morgan Adhesives was closed for the day and the gate to the wellhead and tank battery was closed and locked and I could not access either. I could see a portion of the tank battery. I will follow up in order to establish ownership.
1933601627 2007-03-30 Production Wells No Violations I received a call from the Stow Fire Department reporting a natural gas leak they attributed to this well. My investigation found that there was a gas leak around the wellhead. The well did not have the required identification and the listed owner (KST) is no longer in business. I contacted Marty Tabatcher, Plant Engineer for MacTac and discussed the issues with this as well as the offset well (PN 0579) which MacTac also owns. He said that he would contact their pumper and have the leaking well shut in until repairs could be made. In the mean time, an non-exempt owner packet will be mailed to Mr. Tabatcher so that MacTac can obtain an owner number and properly transfer the two wells.
1973170032 2015-11-04 Final Restoration No Violations I conducted a final restoration inspection of this site. The wellhead and tank battery equipment have been removed. The wellsite has been restored and had no erosion issues. The lease access has also been restored. There is a plug report on file in the state database that shows the well to have been plugged in 2015. Final restoration can be passed.
2030315564 2015-01-14 Plug / Plug Back No Violations Moore Well Service ( MWS ) moved on the well and loaded the hole with 40 barrels brine in order to remove the tubulars in the well. No H2S gas was detected by the monitors on location. I reviewed P & A procedures with MWS personnel and MACtac representatives. No violations.
2081037399 2014-11-05 Production Wells No Violations I was on the Morgan Adhesive # 2 location to observe the dismantling of the tank battery and meet with MACTAC officals, Jeff Moore ( Moore Well Services ) regarding the P & A of the well. The tank Battery consisted of 3 - 100 barrel oil , 1 - 100 barrel brine tank, 2 - mechanical separators and a Hydrogen Sulfide scrubbing unit. The well production is comingled originating from the Newburg Dolomite and Clinton sandstone. This tank battery is also shared with the Morgan Adhesives # 1 ( 34-153-2-0579 ). I did not identify any violations during this inspection.
