0234920876 2017-08-21 Final Restoration No Violations I inspected the plugged and abandoned wellsite and tank battery for compliance in meeting the Final Restoration criteria. The area where the well head was located had all the equipment removed, was graded, had vegetative growth and free of erosional features. The access road had been removed and seeded. The area had been contoured by Pulte Homes for a residential development. A Landowner Waiver had been submitted, by Pulte, to allow the flowline to remain. The area where the well was located meets the criteria for Final Restoration. The designation of Final Restoration is denied due to tank battery which still has two (2) stock tanks remaining and associated piping. The 210 barrel tank had signage identifying the Sunrise Development # 1 ( 34-153-2-1624 ) and Sunrise Development # 3 ( 34-153-2-1623 ) wells. The 100 barrel tank was lying on its side and showing severe evidence of leakage. An inspection of the previous 100 barrel tank location did not show any sign of contamination. The tanks and tank pad removal will have to be completed prior to the Final Restoration status is approved.
0530543869 2017-08-31 Final Restoration No Violations I inspected the Sunrise Development # 1 for compliance with final restoration criteria. I found the 210 barrel and 100 barrel stock tanks were removed. The area was graded, seeded and strawed. Due to lack of vegetative growth for stabilization this tank battery was not in compliance for final restoration designation.
1283196099 2015-09-11 Plug / Plug Back No Violations I inspected this 8.625 casing to determine the cement top height after the previous days plugging. I found the cement top down the 8.625 casing at 98. It had approximately 3 of water on top of the cement. The cement top was firm and set. The well was static, absent of any bubbling. I contacted Petroset Plugging ( P&A Contractor ) and requested that the cement top be brought up to a minimum level of 5. Petroset Plugging mixed five sacks of Sakrete and dumped down the 8.625 casing. This additional cement brought the level up to 42 below the 8.625 casing head. No violations at this time.
1357401643 2017-09-19 Final Restoration No Violations My inspection revealed that the seeding of the tank pad had not completely stabilized the disturbed area at this time. I found a two(2) inch riser on the southwest corner of the tank pad that requires removal. Final Restoration was not approved.
1496419128 2017-06-02 Final Restoration No Violations I received a call from Mark Sparr, representing D.A. Waldron, to inquire of the Final Restoration status of this Clinton Sandstone well that was plugged and abandoned on 9/10/2015. Summit County Inspector Bob Roush conducted a Final Restoration (FR) inspection of this well on 12/5/2016 and FR was not approved due to the presence of fence panels, coiled fencing, and downhole rods near the location of the wellhead requiring removal. This area has been subsequently logged and regraded for residential development and there is no direct access to the wellhead. I parked near a multi-well tank battery and hiked 0.5 miles to the GPS location of the wellhead. My inspection found a pile of stumps and wood with one small piece of fence panel tangled in the debris 20 northeast of the former wellhead. I found no other evidence of fence panels, coils, or downhole rods. I inspected the multi-well tank battery and discovered that the meter run and drip bottle for this well and the Sunrise Development #3 (PN#21623), which is also due for FR approval, remained in a shared fenced area containing a Pin Oak Energy meter run. The tank battery for the Fortuna #s 1 & 4 (PN#s 0541 & 0550) was immediately north of the meter runs. North of the Fortuna battery was a 210 barrel tank posted with a D.A. Waldron ID sign that indicated it was associated with this well and the Sunrise Development #3. North of the 210 barrel tank was a 100 barrel tank on its side that exposed a rotted bottom. This tank was also posted with ID signage indicating association with the #s 1 & 3 and several steel dump lines remained on the ground near the 210 barrel tank. Based on my findings today, Final Restoration cannot be approved. I will speak with Inspector Roush about my findings when he returns from leave. Note: I corrected the Longitude coordinate of the wellhead from .84836 to .48436.
1574906567 2017-11-15 Final Restoration No Violations I inspected the Sunrise Development # 1 to assess the restorative compliance in order to meet Final Restoration criteria. On 12/5/2016, I conducted an inspection after plugging and found various materials still lying on location. These items were removed. On 8/31/2017 a follow up inspection reveal that the tank battery was still partially remaining and needed to be removed and seeded. These items were removed and the area seeded. My inspection on 9/19/17 indicated that while the tanks were removed vegetative growth had not stabilized the disturbed area. The Operator had previously submitted a Landowner waiver to permit the flow lines to remain buried. My inspection today indicated that the vegetative growth had stabilized the area and the area did not reflect any prior oil and gas activities.
1954239610 2016-12-05 Final Restoration No Violations The Sunrise Development # 1 well location was inspected to determine compliance with the Final Restoration requirements. The location was graded and stable. The property was undergoing regrading and construction for a residential community. A Landowner waiver was submitted to DOGRM, by Prestige and Premier Companies ( Pute Homes ), to allow for the sales and flow lines to remain in place after purging. During my inspection , I found that fence panels, coils of chain link fencing and a few downhole rods were left on the location.Until these items are removed the Final Restoration status cannot be assigned.
