1533743408 2018-08-02 Production Wells No Violations The Uniontown office received a request for inspection of the Packer # 1 well located at 2890 Farnham Road, Richfield, OH, 44286. The request was made by Lacy Androne ( 419.516.6543 ). I spoke with Ms. Androne, who indicated that the drinking water was unbearable to consume. A water treatment company supplied test results ( according to Androne ) which showed Total Dissolved Solids (TDS) above 3000 mg/L . Access to the well and tank battery was gained on a residential graveled driveway. The well was operated as an electric pumping well. The well was surrounded with a locked chain link fence. An inspection of the wellhead area did not indicate any leaking pipes or fittings. The stuffing box had grease caked on it from lubricating the polish rod. The valve to the 8.625 X 4.5 annular space was covered and not visible during this inspection. The wellhead was approximately 225 to the residential water well, which was located on the west side of the house. The tank battery (~ 300 from water well) was locked and surrounded with a chain link fence. The I. D. signage was posted and legible. There were two (2) 100 barrel stock tanks and a 2408 horizontal separator in the tank battery. The load line was plugged with a flat plug.. My research indicated that the original tankage capacity, for the Packer #1, was reported to be 310 barrels. This volume of capacity was reduced in 2014 to 200 barrels. The tanks were visually inspection and I did not detect any integrity issues. I did not identify any areas of vegetation which had been scalded or voided of vegetation. Using historical aerial photos it appears that the drilling pits were located on the north side of the well toward the Turnpike. I did not find any surface expression of contamination which would impact the water. I referred Ms. Androne to Scott Scheiner ( Environmental Assessment Program ) for further investigation.
1534338706 2018-08-15 UrbanProduction Wells Violations Noted The Quist # 1 was designated as a Domestic well. The access was gain at 8312 Valley View Road, Macedonia, OH 44056 through the residence driveway. The wellhead was secured with a fence and was unlocked at my request to secure access for the inspection. The vegetation was trimmed and maintained. The well was operated as a plunger lift well with 1.5 tubing inside of 4.5 production casing. The 8.625 surface casing ( from records ) was not visible at the surface. The domestic gas connection point was made at the 4.5 production casing with a regulator, pop - off valve and gauge. The valves were closed on the domestic connection. The gate valve on the 1.5 tubing was closed as was a two (2) inch valve on the flow line. The wellhead was equipped with a motor valve and intermitter control box ( batteries were dead ). There was not any of the required identification signage per OAC 1501:09-9-05 ( E ) 1. I did not identify any leaks or contamination during this inspection at the wellhead. The tank battery was located approximately 100 west of the wellhead. The tank battery was surrounded with a locked ( I requested removal for access ) chain link fence topped with barbed wire. A earthen dike surrounded the tank battery. The vegetation was maintained and cut. The tank battery was comprised of one (1) 140 barrel stock tank, one (1) buried concrete brine vault and one (1) 2408 horizontal separator with orifice meter. A choke manifold, on the inlet side of the separator, was closed. The two (2) inch valve leading from the meter run to sales line was closed. The three (3) inch oil load line , on the stock tank, was locked and plugged, but the two (2) inch brine drain did not have a lock. The brine tank was full. I did not identify any leaks or contamination in the tank battery. The required Urbanized Area ( OAC 1501:9-9-05 ( E ) 1 ) signage was not posted.
