0012379105 2002-04-15 Production Wells No Violations 4-12-2002 - received complaint about stock tank spacing requirements from Margaret Weiner, 1245 Sand Run Rd., Akron, OH., Phone # 330-869-9992, Cell # 330-607-0416. 4-15-2002 - Met with Mrs. Weiner and inspected the site. The tank battery has been moved up near the wellhead and pumping unit. The tank battery is located more than 100 feet from her residence; No Violation. Mrs. Weiner is also concerned about the fence that is no longer around the pumping unit. Further inspection revealed that the separator is only 31 feet from the wellhead and the stock tank is approx. 42 feet from the wellhead. This is a SPACING VIOLATION (spacing rules are included with this N.O.V.) . The separator and stock tank must be at least 50 feet from the wellhead. The stock tank also needs a new I.D. sign. The well name is currently written on the side of the unpainted stock tank. The wellhead and stuffing box are coated with crude and the casing valves are under water and leaking . This is a POLLUTION VIOLATION. Repair the leaks and clean the wellhead and stuffing box. Pine Top will be contacted and informed about the complaint, inspection, and violations. 0048485144 2004-05-12 Production Wells Violations Noted CASE 04-017 The Uniontown office received a complaint from M. Weiner at 1245 Sand Run Road (330-869-9992) about a petroleum odor and runoff from the well on her neighbors property. She was also concerned about a large pond located down gradient from the tank battery. When I arrived, I found approx. 2.5 feet of water and oil inside the dike. I contacted Tony Carlisle with Pine Top and met with him on site within 15 minutes of my arrival. According to Tony, the brine tank floated last winter and broke a valve on the 210 bbl. They cleaned the spill without reporting it to MRM. Apparently the same thing happened again, with the loss of oil and brine into the containment dike. Tony contacted Bob Moore and arranged for a vacuum truck to recover oil and water from inside the dike. I also instructed Tony to remove the equipment from the tank battery, remove and dispose of impacted soil and gravel, replace the 210 bbl tank and reconstruct the tank battery. The well and tank battery also lack the required ID.

0920165156 2006-06-29 Production Wells Violations Noted I received a call from Kurt Kollar with OEPA reporting an oil spill originating from this tank battery. The spill has impacted two downgradient ponds. According to Tony Carlisle with Pine Top, the pumpjack timer failed and the pump ran four feet in the tank which overflowed the tank and ran through the containment dike. The dike would have worked if Pine Top had not cut drainage slits in the dike. The loss was estimated at two barrels of oil. Since the spill reached water, the initial response was directed by the OEPA.
1017282018 2019-04-25 Production Wells Violations Noted My inspection found the well is equipped with pumpjack. The well last produced in 2015 for 295 MCF of gas. There was zero production for 2016 and there is no Annual Production Report for 2017. The tank battery consists of a 100-barrel tank, a steel brine tank, a vertical separator and a meter. The well is not legibly identified. The well is idle. Consequently, the well is in violation of Revised code 1509.062 and shall be plugged in accordance with Ohio Revised Code section 1509.12 (B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1152751562 2006-07-06 Production Wells Violations Noted The impacted area around the stock tank has been removed and fresh stone placed in the tank battery. The containment dike has been repaired and fortified. The large siphon dam is holding and contains a sheen. The small siphon has a heavy sheen and several sorbent pads.

The fluids were transferred to other Pine Top tank batteries. On this date, the containment floor was being washed again and miscellaneous equipment was removed. According to Tony, the 210 will be cleaned and removed, the remaining equipment will be cleaned and removed and the impacted soil removed on Monday 5/17.
2001869914 2004-06-09 Production Wells Violations Noted Spill remeidation has been completed. The tank battery still needs to be put back together and the leaking 210 replaced. Progress has slowed since the initial response. I will contact Tony Carlisle for a completion date.
