0210208539 | 2018-10-11 | UrbanProduction Wells | Violations Noted | The access road was graveled to the tank battery and wellhead. The well was operated as an electric pumping well. The well was fenced, but not locked. Legible I.D. signage was posted at the wellhead. No leaks or issues were identified at the wellhead. The tank battery was surrounded with a locked chain link fence and earthen dike. The tank battery was grossly overgrown with vegetation which impeded inspection of the equipment. The tank battery was comprised of one (1) 210 barrel stock tank, one (1) in ground concrete brine vault and one (1) 2408 horizontal separator with orifice gas meter. The meter had a chart which reflected the current production period. I.D. signage was posted on the meter house door. The 210 barrel stock was leaking brine out of the heater tube, which was in violation of OAC 1501:9-9-05 A(8 ) which states in part all surface equipment must be pressure rated to withstand operating pressures to which it is subjected. The overgrown vegetation was in violation of OAC 1501:9-9-05-E(5)e, which states that Equipment and vegetation shall be maintained consistent with prudent operation,. In order to bring the Vincent #3 well into compliance the Owner/Operator shall replace the leaking 210 barrel stock, remove any contaminated material below the heater tube and cut the overgrown vegetation in order to not impede access. The Owner/Operator shall contact Bob Roush @ 330.690.8837 at least 24 hours prior to the compliance work starting to allow his attendance during remediation. The Vincent #3 shall be brought into compliance and approved, by DOGRM Inspector, by 11/2/2018. |

The tank battery was rebuilt utilizing a 100 barrel stock tank and the contaminated stone below the tank was removed and replaced with fresh material. The faulty 210 barrel tank was placed on containment and remains on location. The contaminated pad material was encapsulate and remains on location. I spoke with Zack Moore who said, that the tank would be removed by 12/14/18. The Well Owner was responsible to removed the stone tank pad material. The requirements of the Compliance Notice # 210208539 have not been completely met and remains in effect. |
