0355694831 2003-04-25 Production Wells No Violations FOLLOW UP TO NOV # -1634458278 The Uniontown office received a call from Todd Dunn (304-324-2417) concerning the status of this well. The last reported production to ODNR was 1993. The last revenue dispursed to Mr. Dunn was 2001. The tank battery includes one 210, one 100 bbl and one horizontal separator. The well is operated with a pump jack. The contract pumper listed on the ID has not operated the well for over a year. According to John McNally with Whidbey, the well has not produced because of a bad sales line. Furthermore, the well is being sold to North American Petroleum (Rick Batta 330-262-5865) who reportedly intends to make the necessary repairs and place the well back into production.

1118696078 2018-09-04 UrbanProduction Wells Violations Noted The access road was overgrown and did not allow vehicle traffic. The tank battery was fenced, but left unlocked ( I re-locked it ) . The I. D. signage was not legible. The tank battery was comprised of one (1) 210 barrel stock tank, one (1) 100 barrel brine tank and a horizontal separator. The inside of the tank battery impeded a thorough inspection due to the overgrown vegetation. The load line was plugged. The 210 barrel stock tank was leaking at the bottom seam ( south side ) and causing ground contamination. The 210 barrel also had holes at approximately the 12 foot level also on the south side. The orifice gas meter had a illegible paper chart attached. Production was reported for the year 2016 at zero (0) volumes of gas, oil or brine. The production year of 1993 was the last year of production. The well was operated as an electric pumping well. The electric meter did not have a meter set. The wellhead was fenced and locked. The inside of the wellhead fence was overgrown and did not permit access. I. D. signage was posted. The wellhead control valves were in the closed position. I did not located any leaks at the wellhead during this inspection. Whidbey Resources Inc. shall 1) submit, within two (2) weeks, a schedule for the plugging of the Chemionics # 1 to Bob Roush at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio, 44685 for approval. Whidbey Resources Inc. will be notified if the plugging schedule is accepted and shall plug the well within 180 days, 2) post the required Urbanized Area compliant signage , 3) remove all fluids from the tanks located in the tank battery, 4) remove and replace the leaking 210 barrel stock tank, 5) cut all vegetation growing inside of the tank battery and wellhead as to allow access. Compliance items # 2, # 3, #4 and #5 shall be completed and approved within 30 days of this notice ( 10/6/2018 ). 1232501349 2009-01-15 Production Wells No Violations No change in the status of this well. It is still idle with the last reported production in 1993. Several different operators have expressed an interest in obtaining and operating this well but no transfers have ever been submitted to Columbus.
1480881627 2003-09-02 Production Wells Violations Noted John McNally with Whidbey stated in May that this well was being sold to North American Petroleum and that the sale would be completed soon. On June 24, I received notice that six Whidbey wells had been transferred to North American. The Chemionics well was not on that list. The well must be plugged or produced.
